精神分裂症综合治疗的临床及认知功能评估

摘　　要：目的探讨不同维持治疗方法对门诊精神分裂症患者临床及认知功能的影响情况。方法140例缓解期患者随机分为单纯药物治疗组(单纯组)和药物治疗结合社会心理康复训练组(综合组)以及典型与非典型药物治疗组,治疗期采用威斯康星卡片分类(WCST)、连续作业测验(CPT)、修订韦氏成人智力量表(WAIS-RC)及复发率进行纵向临床评定,随访18个月。结果单纯组复发率(42.9%)比综合组(18.6%)高(P<0.01),典型药物组复发率(44.0%)比非典型药物组(23.3%)高(P<0.05)。经LOCF分析,综合组WCST、CPT、WAIS-RC等认知功能测验项目比单纯组好(P<0.05,P<0.01)。非典型药物组上述测验项目比典型药物组好(P<0.05,P<0.01)。结论药物结合社会心理康复训练以及非典型药物维持治疗对降低精神分裂症患者复发、改善其认知功能均有一定效果,综合治疗效果更明显。Objctive To explore the health outcomes of the diference rehabilitation intervention in schizophrenia outpatients. Methods 140 outpatients with stable Schizophrenia,who were given of antipsyehotic drugs were randomly divided into comprehensive intervention group and control group,typical and atypical antipsychotics group. All patient were evaluted with Wisconsin card sort test（WCST）,Continuous performance test（CPT）,Wechsler adult intelligence scal（WAIS-RC）and the frequency of relapse, in 18 months follow-up study. Results At the 18 months following point,the frequency of relapse of patients in control group（42. 9 % ） than Comprehensive intervention group （ 18.6 % ） was higher（ P〈0.01 ）, typical antipsychotics （44.0% ） than atypical antipsychotics group（23. 3%）was higher （P〈 0. 05）. The WCST, CPT, WAIS-RC in Comprehensive intervention group than control group,atypical antipsychotics than typical antipsychotics group was better（P〈0.05 and P〈0.01） With last observation carried forward（LOCF）. Conclusion Comprehensive intervention and atypical antipsychotics contribute to the improvement of social function and the reducing relapse of Schizophrenia outpatients.
