摘 要:【目的】观察比较不同方法注射透明质酸酶(HA)诱导兔眼基底部玻璃体脱离。【方法】取新西兰白兔105只,随机分6组,其中A、B、C每组30只,D、E、F每组5只。A、B、C各组一眼基底部玻璃体注射HA0.05mL,浓度分别为250、500、1000U/mL,另一眼相同部位注射等量平衡盐液体;D、E、F各组常规玻璃体腔注射HA0.05mL,浓度分别为250、500、1000U/mL,另一眼相同部位注射等量平衡盐液体。于注射后6h、1d、3d、7d、14d分别行裂隙灯、UBM检查,随机分批处死兔,取眼球标本行HE染色及基底部扫描电镜、透射电镜检查,观察基底部残余玻璃体。【结果】病理观察A组实验眼在3、7、14d的脱离率分别为20%、20%、40%;B组为20%、40%、80%;C组为40%、60%、80%;UBM显示A组实验眼在各时间点脱离率为0;B组在14d的脱离率为33.3%;C组在第7天和14天的脱离率为33.3%和66.7%,D、E、F组观察2周,未见基底部玻璃体脱离。各组均未发生眼内炎性反应和视网膜毒性反应。【结论】局部注射透明质酸酶可成功诱导基底部玻璃体脱离。[Objective] To investigate and compare basal vitreous detachment induced by hyaluronidase (HA) in different administration routes in rabbits. [Method] A total of 105 New Zealand white rabbits were randomly assigned to 6 groups: group A, B, and C with 30 rabbits in each group, and group D, E, and F with 5 rabbits in each group. The rabbits in group A, B, and C were given HA 0.05 mL at the concentration level of 250, 500 IU/mL, and 1 000 IU/mL by vitreous base injection in one eye, respectively ; and the same amount of equilibrium liquid was given to the other eye by the same route. Rabbits in group D, E, and F were given HA 0.05 mL at the concentration level of 250, 500, and 1 000 IU/mL by vitreous cavity injection in one eye, respectively; and the same amount of equilibrium liquid to the other eye by the same route. Slit lamp examination and ultrasound biomicroscopy (UBM) were carried out at 6 h, 1 d, 3 d, 7 d, and 14 d after the injections. After that, the rabbits were executed. Residual vitreous base was detected. The eye ball samples were obtained and observed under scanning electron microscopy (SEM) and transmission electron microscopy (TEM) after HE (Haematoxylin & Eosin) staining. [Result] In group A, the detachment rates at 3, 7, and 14 d were 20%, 20%, and 40% by pathologic examination, respectively; in group B, the detachment rates were 20%, 40%, and 80%, respectively; and groups C 40%, 60%, and 80%, respectively. UMB examination revealed that the detachment rates at all time points in group A were 0; the detachment rate was 33.3% at 14 day in group B, and for group C, the detachment rates were 33.3% and 66.7% at 7 d and 14 d, respectively. For group D, E, and F, no detachment was detected within 2-week observation. No intraocular inflammation or retinal toxicity was seen in all the eyes of all groups. [ Conclusion] Vitreous base HA injection significantly induced basal vitreous detachment.
