META ANALYSIS OF CURATIVE EFFECT ON ANGLE CLASS Ⅱ MALOCCLUSION WITH FUNCTIONAL APPLIANCE

摘  要:[目的]分析排除了自然生长因素后,功能矫治器治疗安氏Ⅱ类错牙合畸形引起的下颌骨的改变,从而更为真实、准确地评价该类矫治器的矫治效能。[方法]运用Meta分析技术对功能矫治器用于矫治生长高峰前期或高峰期安氏Ⅱ类错牙合畸形患者矫治前后SNB角、AR-GO、AR-PO、GO-GN进行综合定量分析。根据资料异质性检验,均采用随机效应模型。计算合并SMD及其95%CI。[结果]①资料的异质性检验:头影测量指标SNB角、AR-GO、AR-PO、GO-GN异质性检验P值都小于0.05,具有异质性。②Meta分析结果:AR-PO、AR-GO、GO-GN、SNB角的合并SMD均大于0,结果均具有统计学意义,分别是1.34(0.58~2.10),1.02(0.55~1.49),1.10(0.62~1.59),1.03(0.76~1.29)。[结论]功能矫治器明显增加下颌骨总长度、下颌升支、下颌体长度。使颏部前移,SNB角减小。[ Objective ] To evaluate the mandibular Changes produced bv the functional appliance excluding the influence of natural growth, thereby, it is more objective, actual and accurate to reflect the treatment effect. [Methods] Meta-analysis was used to make formal summary statements about the effect of functional appliance used in the late mixed, or preadolescent dentition with cephalometric analysis such as AR-PO AR-GO, GO-CN, SNB. According to homogeneity test, all factors adopted random effect model. Random effect model should be selected to calculate total pooling SMD and its corresponding 95% confidence interval (95%CI). [ Results ] ①Homogeneity test showed that all factors (P≤0.05) were significant heterogeneity. ② Result of meta-anaJysis the total pooled SMD value (95%CI) of AR-PO, AR-GO, GO-GN, SNB was above 0, which was 1.34 (0.58-2.10), 1.02 (0.55-1.49), 1.10 (0.62-1.59), 1.03 (0.76-1.29) respectively. AH showed significant staffstical difference. [Conclusion ] The functional appliance significantly increase the mandibular unit length, the ramus height and the mandibular body length. The chin moves forward, which increases the SNB degree.
