阿霉素与无水乙醇配合心理疗法治疗三叉神经痛的临床观察

摘　　要：目的观察阿霉素与无水乙醇经卵圆孔半月神经节阻滞及配合心理疗法治疗三叉神经痛的疗效。方法将63例原发性三叉神经痛患者随机分为A、B两组,A组31例患者采用无水乙醇0.3ml进行半月神经节阻滞;B组32例患者采用0.5%阿霉素0.3ml进行半月神经节阻滞,两组均配合使用心理治疗。分别观察1周、1年后疗效以及治疗前、后疼痛评分和焦虑、抑郁情况。结果经治疗1周、1年后A组优良率分别为93.5%、61.3%;B组优良率分别为90.1%、81.3%,治疗1周后差异无统计学意义(x2=2.06,P>0.05),1年后差异有统计学意义(x2=12.47,P<0.05),两组均无严重并发症。两组治疗前后VAS、SAS、SDS评分差异有统计学意义(P<0.05)。结论阿霉素与无水乙醇半月神经节阻滞配合心理疗法治疗原发性三叉神经痛皆有效,但阿霉素的疗效优于无水乙醇。把疼痛与情绪障碍结合起来,进行综合的心理及药物治疗更有利于改善患者的病情。Objective To observe the therapeutic effects and adverse effects of adriamycin and dehydrated alcohol with psychotherapy for the treatment of primary trigeminal neuralgia. Methods Sixty three patients with primary trigeminal neuralgia were divided into two groups randomly. A group （n =31 ） was given mutilation of gasserian ganglion by dehydrated alcohol O. 3ml and B group （ n = 32 ） was given by 0. 5% adriamycin O. 3ml. Psychotherapy was used in two groups respectively. The parameters included therapeutic effects at lw, 12m after treat- ment, also included visual, analog scale （ VAS）, self - rating anxiety scale （SAS） , self - rating depression scale （SDS） before and at l w, 12m after treatment. Results The pain intensity in two groups decreased significantly at lw, 12m after treatment. There was no significant statistic difference between the decrease of pain intensity of two groups at lw after treatment（ x 2 =2.06, P 〉0. 05 ）. But there was a significant statistic difference at 12m after treatment（ x 2 = 12.47, P 〈 0. 05）. Also there was a significant statistic difference between the VAS ,SAS ,SDS total scores of two groups before and after treatment, respectively （ P 〈 0.05 ）. Conclusion It is effective to use adriamycin and dehydrated alcohol with psychotherapy for the treatment of primary trigeminal neuralgia. But adriamycin is more suitable for trigeminal neuralgia than dehydrated alcohol The conclusion imply that it is more effective to use medicine with psychotherapy for the treatment of primary trigeminal neuralgia.
