动态监测血乳酸和肌钙蛋白I在早发型重度子痫前期中的价值探讨

摘　　要：目的:探讨血乳酸和肌钙蛋白I变化规律在早发型重度子痫前期疾病中的价值。方法:对27例早发型重度子痫前期孕妇血乳酸和肌钙蛋白I含量进行动态监测,与同孕龄正常妊娠对照组进行比较。结果:(1)早发型重度子痫前期组血乳酸含量较同孕龄对照组有显著性差异(P<0.01);动态监测发现早发型重度子痫前期患者随子痫抽搐、引产、剖宫产手术和HELLP综合征、DIC的发生,血乳酸水平明显升高,血乳酸含量随机体损伤加重而增加;(2)动态监测发现早发型重度子痫前期1例患者,肌钙蛋白I含量随病情加重逐渐增高,入院含量为0.27ng/ml,随病情加重逐渐增高至1.4ng/ml维持数日,并出现心电图异常改变。另有2例患者入院时在(0.31～0.40 ng/ml)之间维持数日,未出现心电图异常改变。结论:动态监测血乳酸和肌钙蛋白I含量,可作为早发型重度子痫前期疾病诊疗及严重程度的监测指标和患者心肌细胞受损的理想指标。Objective：The study focuses on the value of the changing rules of blood lactic acid and troponin I in the early onset of severe pre-eclampsia. Methods： The contents of blood lactic acid and troponin I of 27 pregnant women in the early onset of severe pre-eclampsia are dynamically monitored,and the results are compared with those of the control group of normal pregnancy. Results： （ 1 ） Significant difference （P 〈 0. 01 ） has been found between the results of the two groups ; it has also been found that blood lactic acid rises with the occurrence of convulsions,induction surgery,cesarean section operation, HELLP syndrome and DIC. The level of blood lactic acid rises significantly and the content of lactic acid rises with the severity of body damage. （2） In one case with the early onset of severe pre-eclampsia, the content of troponin Ⅰ increases with the exacerbation of the illness. The content at admission was 0.27ng/ml, later increased to 1.4ng/ml with the exacerbation, and remained like this for days, but without abnormal change in the electrocardiogram. The contents of anotber two cases stayed between 0. 31 and 0. 40ng/ml at admission and remained like this for days, but without abnormal change in the electrocardiogram. Conclusions ： Dynamically monitoring the con- tents of blood lactate and troponin Ⅰ can be used as the monitoring indicator for the diagnosis and severity of the early onset of severe pre-eclampsia. Troponin Ⅰ can be the ideal indicator for judging the myocardial cell damage of the patients with early onset of severe pre-eclampsia.
