摘  要:目的:调查白内障防盲手术患者的人工晶体置入情况,使人工晶体配置更加合理。方法:纳入在湖南桑植县进行的159例白内障防盲手术患者,记录人工晶体置入前度数测量值、术中置入度数和患者置入后1d的视力。比较置入前患者人工晶体度数测量值及术中置入人工晶体度数真实值的差异,观察置入晶体度数差值较大的患者情况。结果:白内障防盲手术前测量人工晶体最小度数为3.0D,最大为29.0D。<10D人工晶体占5.0%,呈不连续分布;10D≤人工晶体度数≤14D占5.0%;15D≤人工晶体度数≤17.5D占5.0%;18D≤人工晶体度数≤19.5D占10.0%;20D≤人工晶体度数≤23.5D占62.3%;24D≤人工晶体度数≤26.5D占8.8%;人工晶体度数>26.5D占3.8%。其中以21D,21.5D和22D最多,频率分别为12.6%,14.5%和12.6%。术中置入的人工晶状体<10D的占6.3%;11D≤人工晶体度数≤14D占3.8%;15D≤人工晶体度数≤17.0D占3.8%;18D≤人工晶体度数≤19.5D占10.7%;20D≤人工晶体度数≤23.5D占63.5%,其中21.5D频率最高为14.5%;24D≤人工晶体度数≤26.5D占10.7%;人工晶体度数>26.5D缺如。术中置入人工晶体度数与置入前测量值的差值完全符合(差值为0)者占69.2%;相差值为0.5D者占13.8%,1~3D者占12.0%,>3D者5.0%。差值≥3.0D的11例患者置入后10例双眼呈低视力状态。结论:在防盲行动中人工晶体总量及常用度数晶体量的配置基本合理,<17D和>23.5D的人工晶体配置欠合理,缺如大部分≤10D的人工晶体,应进一步改善人工晶体的配置方案和方法。OBJECTIVE: To investigate the diopter value of intraocular lens (IOL) used in cases in order to provide an optimizing IOL diopter proportion. METHODS: A total of 159 cases with cataract from Sangzhi County, Hunan Province were studied. Their IOL diopter values were measured prior to operation, diopter values of IOL practically transplanted, and visual acuity on the first day after operation were recorded. Difference between measured and transplanted lens diopter was compared, and the conditions of the cases implanted more difference lens were overviewed. RESULTS: The lowest IOL diopter value measured before operation was 3.0 D, the largest one was 29.0 D. The frequency of different diopter values: 〈10 D, 5%; 10 D ≤IOL≤14 D, 5%; 15 D≤IOL≤17.5 D, 5%; 18 D≤IOL≤19.5 D, 10%; 20 D≤IOL≤23.5 D, 62.3%; 24 D≤IOL≤26.5 D, 8.8%; IOL〉26.5 D, 3.8%. Among them IOL of 20 D, 21.5D and 22D were the higher with probability of 12.58%, 14.47% and 12.58%. The component proportions of IOL diopter transplanted were: 〈10 D, 6.29%; 11 D≤ IOL≤14 D, 3.77%; 15 D≤IOL≤17.0 D, 3.77%; 18 D≤IOL≤19.5 D, 10.69%; 20 D≤IOL≤23.5 D, 63.52%; 24 D≤IOL≤26.5 D, 10.69%; no IOL of 26.5 D. Among all the cases, there was 69.18% with no difference found between measured diopter and that of transplanted, 13.83% with difference between 0.5 D, 11.95% 1-3 D and 5% above 3 D. Among the 11 cases with 3.0 D difference or above, 10 cases were lower vision after operation. CONCLUSION: The numbers of total IOL series and frequently used IOLs prepared before operation are reasonable. The distributions of IOLs diopters which 〈17 D or 〉 23.5 D were not optimize. Most IOLs with diopters ≤10D were absence. Therefore, it is necessary to improve the distribution of intraocular lens diopter in cataract surgery for prevention of blindness.
