1152例恶性血液病回顾性研究

摘　　要：目的回顾性分析我院10年间初发恶性血液病的情况,以研究其发病情况及流行趋势。方法取骨髓片及血片作瑞氏染色,细胞化学染色,然后显微镜读片依据FAB标准作出诊断。结果1998年1月至2007年12月初发恶性血液病1152例。其中急性淋巴细胞白血病(ALL)269例,占23.35%;急性非淋巴细胞白血病(ANLL)409例,占35.5%;慢性粒细胞白血病(CML)161例,占13.98%;慢性淋巴细胞白血病(CLL)18例,占1.56%;浆细胞白血病(PCL)5例,占0.43%;多发性骨髓瘤(MM)91例,占7.90%;骨髓增生异常综合征(MDS)141例,占12.24%;其它58例,占5.03%。在0～9岁年龄组,ALL为首位占67.63%(94/139),在30～39岁年龄组ANLL的发病比例达到53.29%(89/167),CML发病在20岁以上明显增加,CLL的发病多在50岁以上,MM和MDS的发病在40岁以上有明显增加的趋势,在>60岁年龄组比例最高,分别为60.44%(55/91)和57.45%(81/141)。1998-2007年构成比依次为8.07%、8.25%、9.38%、8.07%、7.38%、10.68%、10.94%、10.24%、13.54%、13.45%。结论近年恶性血液病有增加的趋势。Objective To perform retrospective analysis of malignant hematological diseases reported in this hospital. Methods Samples of bone marrow smears and blood smears between Jan 1998 and Dec 2007 were retrospectively analyzed. The smears were stained using cytochemical and Wright＇s staining methods. The stained cells were then microscopically examined and analyzed according to the FAB standard. Results A total of 1,152 cases of malignant hematological diseases were found. The incidence rate of ALL, ANLL, CML, CLL, PCL, MM, MDS and others was 23.35% （269/1152）, 35.5% （409/1152）, 13.98% （161/1152）, 1.56% （18/1152）, 0.43% （5/1152）, 7.90% （91/1152）, 12.24% （141/1152） and 5.03% （58/1152）, respectively. In the age group of 0-9, the percentage of ALL was 67.63%. For the age group of 30-39, the incidence rate of ANLL was 53.29% （89/167）. The incidence rate of CML and CLL was obviously high in the patients with an age of 20 and 50, respectively. The incidence of MM and MDS was increasing in patients with an age of 40 and the incidence reached the peak at the age of 60 （MM, 60.44% and MDS, 57.45%）. The incidence rate of malignant hematological disease from 1998 to 2007 was 8.07%, 8.25%, 9.38%, 8.07%, 7.38%, 10.68%, 10.94%, 10.24%, 13.54% and 13.45%, respectively. Conclusion There was a tendency of increase in malignant hematological disease in the past 10 years.
