胰岛素样生长因子-1受体在恶性黑色素瘤中的表达及临床意义

摘　　要：目的探讨恶性黑色素瘤组织中胰岛素样生长因子-1受体(insulin-like growth factor-1 receptor,IGF-1R)的表达及其临床意义。方法选取经病理检查明确诊断的恶性黑色素瘤石蜡切片26例(恶性黑色素瘤组)及正常色素痣石蜡切片20例(正常色素痣组)。采用EnV ision两步法,以兔抗人IGF-1R多克隆抗体检测上述两组IGF-1R的表达情况,并分析恶性黑色素瘤组织中IGF-1R的表达与年龄、性别及有无远处转移的关系。结果恶性黑色素瘤组IGF-1R的阳性表达率为57.7%(15/26),正常色素痣组为25.0%(5/20),两组比较差异有统计学意义(χ2=4.912,P=0.023);恶性黑色素瘤组IGF-1R的高表达与有无远处转移相关(P=0.038),而与性别、年龄无关(P=0.453,P=0.701)。结论 IGF-1R的表达量可以预测原发性恶性黑色素瘤的转移及预后,对恶性黑色素瘤的治疗可能具有一定的指导意义。Objective To investigate the expression and clinical significance of the insulin-like growth factor-1 receptor(IGF-1R) in malignant melanoma.Methods 26 cases of paraffin sections diagnosed as malignant melanoma by pathological examination(malignant melanoma group) and 20 paraffin sections of normal pigmented nevus(normal pigmented nevus group) were selected.With En Vision two-step method,the expression of IGF-1R in the two groups was detected by rabbit polyclonal anti-human IGF-1R antibody,and relationship of IGF-1R expression with age,sex and distant metastasis was analyzed.Results The positive expression rate of IGF-1R in malignant melanoma group was 57.7%(15/26),significantly higher than that in normal pigmented nevus group(25.0%,5/20),the statistical analysis showed significant difference(χ2=4.912,P=0.023);the high IGF-1R expression in malignant melanoma group was related with distant metastasis(P=0.038),but not with sex and age(P=0.453,P=0.701).Conclusion IGF-1R expression level can predict the metastasis and prognosis of primary malignant melanoma,and may have some significance for its treatment.
