瞬时高眼压对兔视网膜BDNF/TrkB浓度影响的研究

摘  要:目的观察准分子激光原位角膜磨镶术(LASIK)术中负压吸引对兔视网膜BDNF/TrkB浓度的影响,从神经营养因子被剥夺的角度分析LASIK术中负压吸引引起视网膜神经节细胞(RGCs)凋亡的发生机制。方法 48只健康新西兰大耳白兔随机分为实验对照组、负压吸引20s组、吸引45s组和吸引3min组。实验对照组只进行激光治疗,负压吸引各组先用负压发生器吸引眼角巩膜缘,持续20s、45s及3min,分别于术后即刻、7d、10d、14d各时间点处死白兔,取视网膜组织检测脑源性神经营养因子(BDNF)及其受体TrkB的浓度。结果视网膜BDNF/TrkB浓度的动态观察:与实验对照组相比,负压吸引20s和45s组视网膜BDNF/TrkB的浓度变化无统计学意义(P>0.05)。而负压吸引3min组视网膜BDNF和TrkB的浓度都有所增加,与实验对照组差异有统计学意义(P<0.05),且都于术后7d达到最高峰,10d则明显下降,14d回到接近正常对照组水平。负压吸引3min后各时间点BDNF和TrkB的浓度变化与实验对照组比较,术后即刻和7d差异有统计学意义(P<0.05),术后10d和14d差异无统计学意义(P>0.05)。结论 LASIK术中负压吸引引起的眼压急剧升高,可以导致视网膜中BDNF/TrkB浓度变化,但这种改变为可逆的;负压吸引持续时间越长改变越明显。神经营养因子被剥夺是高眼压引起RGCs凋亡的机制之一。Objective To observe the influence of transient high IOP by negative pressure suction during LASIK on BDNF and TrkB concentration in rabbit retina,and analyze the mechanism of RGCs apoptosis that caused by transient high IOP during LASIK.Methods 48 New Zealand white rabbits were randomized to experimental control,negative pressure suction 20 s,negative pressure suction 45 s and suction 3 min groups.The experimental control was treated only by laser.Rabbit eyes received suction for different periods of time(20 s,45 s,3 min) by negative pressure generator in different groups.Then the rabbit were executed at different time points of post-LASIK,such as postoperative instant,7 d,10 d,14 d,finally,retinal tissue was extracted to detect the concentration of BDNF and TrkB.Results The concentration of BDNF and TrkB dynamic observation:there were no statistical significance on concentration of BDNF and TrkB in negative pressure suction 20 s and 45 s groups compared with experimental control(P > 0.05).There were obvious increase on the concentration of BDNF and TrkB after suction 3 min compared with control(P < 0.05),and the concentration of BDNF and TrkB reached peak at postoperative 7 d,decreased at postoperative 10 d,returned to normal level at postoperative 14 d.Compared with experimental control,there were statistical significance on concentration of BDNF and TrkB at postoperative instant and 7 d in suction 3 min(P < 0.05),there were no statistical significance at postoperative 10 d and 14 d(P > 0.05).Conclusions The high IOP caused by negative pressure suction during LASIK could result in the change of the concentration of BDNF and TrkB,but it is reversible.The longer the duration,the more obvious the change.Neurotrophic factor being stripped is one of the mechanism of RGCs apoptosis.
