摘 要:目的明确CST1在膀胱移行细胞癌(BTCC)组织中的表达水平及其对膀胱癌患者复发的影响。方法通过荧光定量PCR方法对28对BTCC患者标本(包括癌组织和癌旁正常组织)的CST1mRNA进行分析;并运用免疫组化对16对患者癌组织及其癌旁正常组织和85例癌组织切片进行染色;运用统计学方法分析CST1的表达量与患者的临床特点之间的关系。结果荧光定量PCR显示膀胱癌组织中CST1表达水平明显高于癌旁正常组织(P<0.001);16对患者切片免疫组化显示,癌组织CST1表达水平较正常组织高;85例癌组织染色结果显示:CST1的表达水平与肿瘤直径(P<0.001)、病理分期(P<0.001)、分级(P=0.016)和复发率(P<0.001)相关;单变量分析显示CST1表达水平、肿瘤大小、病理分级、分期均是BTCC复发危险因素,二元非条件Logistic回归分析显示CST1表达高低是影响患者复发的独立因素(OR=9.351,P<0.001)。结论 CST1的表达水平和BTCC复发之间存在明显相关性;研究发现CST1表达上调的患者复发率高,CST1可能是一个新颖并且有临床价值的预测膀胱癌复发的生物学标记物。Objective To identify the level of CST1 in bladder translational cell carcinoma(BTCC)and analyze its clinical significance on the prognosis of BTCC patients.Methods We examined the expression of CST1 mRNA by real time RT-PCR in 28 paired BTCC and the adjacent normal tissues respectively.The expression of CST1 protein in 85 specimens of BTCC and 16 paired adjacent normal tissues and carcinoma tissue were determined by immunohistochemistry.Statistical analyses were applied to test the relationship between the expression of CST1 and clinical features and recurrence.Results The mRNA level of CST1 was significantly higher in BTCC tissues compared with the adjacent normal tissues by real time RT-PCR(P<0.001).We analysis 85 BTCC specimens by immunohistochemistry and 16 paired tissues.Accordingly,the CST1 protein expression was higher in BTCC tissues compared with the adjacent normal tissues(P<0.001).Moreover,there was a significant correlation between CST1 expression in BTCC and tumor size(P<0.001),Pathologic stage(P=0.016),T stage(P<0.001)and recurrence(P<0.001).Multivariate analysis indicated that CST1 protein upregulation was an independent predictor for high probability of recurrence of bladder cell carcinoma patients.Conclusions Our results indicate that CST1 may represent a novel recurrence marker of bladder cell carcinoma.Patients with higher BTCC expression had greater probability of recurrence than those with lower CST1 expression.Measurement of CST1 expression in primary BTCC can help to stratify the patients for follow-up.
