早期肠内营养对胃癌患者术后细胞免疫功能和肿瘤标志物的影响

摘　　要：目的探讨胃癌病人术后早期肠内营养对机体细胞免疫力及肿瘤标志物的影响。方法选择胃癌根治性手术病人42例,随机分两组:早期肠内营养组21例,于术后24小时给予能全力,共8天;普通肠内营养组21例。两组病人分别于术前1d、术后第1天、术后第9天检测免疫学指标:CD3+、CD4+、CD8、CD4+/CD8+比值、sIL-2R;肿瘤标志物CEA、CA19-9、CA125。结果术后第一天两组病人免疫学指标:CD3+、CD4+、CD8、CD4+/CD8+比值、sIL-2R;肿瘤标志物CEA、CA19-9、CA125。均有所下降,而术后第9天,早期肠内营养组患者免疫学指标:CD3+、CD4+、CD8、CD4+/CD8+比值迅速恢复,sIL-2R浓度迅速降低,肿瘤标志物CEA、CA19-9、CA125下降明显,与普通肠内营养组比较差异均有统计学意义(P<0.05)。结论早期肠内营养支持能促进胃癌术后病人的细胞免疫力恢复,进而增强患者机体抗肿瘤能力。Objective To investigate the effect of early enteral nutrition(EEN) on immune function and tumor marker in surgical patients with gastric cancer.Methods Fifty patients with gastric cancer surgical patients were randomly divided into EEN group and routine enteral nutrition(REN) group.The patients in EEN group received Nutrison Multi Fibre early enteral nutrition from postoperative day 1 to postoperative day 8.The patients in REN group received Nutrison Multi Fibre after Passage gas anus.CD3+,CD4+,CD8,CD4+/CD8+,sIL\|2R,CEA,CA125,CA19\|9 were detected on the day before the operation and the postoperative days 1 and 9.Results As compared with the day before the operation,postoperative day 1,all the levels of CD3+,CD4+,CD4+/CD8+,CEA,CA19\|9,CA125 in two groups were decreased,and sIL\|2R was elevated.However,postoperative day 9,the changes in EEN group was significantly different from REN group(P<0.05).Conclusion Compared with the routine enteral nutrition,early enteral nutrition can improve obviously immune function and the ability of antitumor in post operative patients with gastric cancer.
