摘  要:目的探讨核素骨显像与肿瘤标志物 CYFRA21-1、CEA 对肺癌骨转移的临床诊断价值。方法对216例肺癌患者进行单光子发射计算机断层仪(SPECT)全身骨显像的同时,进行血清CYFRA21-1、CEA 的检测,另选择肺部良性疾患患者20例进行对照。结果 216例肺癌患者中121例出现骨转移,阳性率56.02%,多发病灶(101个)多于单发病灶(20个),骨转移患者血清 CYFRA21-1、CEA 浓度及阳性率明显高于无骨转移和肺部良性疾患组,骨转移和骨未转移组与良性病变组之间CYFRA21-1、CEA 检测结果差异有统计学意义(P<0.05)。结论肺癌患者在行全身骨显像时联合检测CYFRA21-1、CEA 对其临床分期、疗效判断有重要的临床意义。Objective To evaluate the value of CYRTA21-1,CEA and SPECT in diagnosis of bone metastasis in lung cancer.Methods 216 patients with lung cancer were examined by radionuclide bone imaging and serum CYFRA21-1,CEA.20 patients with benign lung disease were also studied.Results 121 of the 216 lung cancer patients were confirmed bone metastasis including 101 multi-metastasis and 20 singu- lar-metastasis. The positive rate was 56.02 %.The serum CYFR21-1,CEA level and positive rate of lung cancer patients with bone metastasis were obviously higher than those of patients without bone metastasis or with benign lung disease.The level of serum CYFR21-1,CEA had significant difference(P<0.05)between the positive group of bone imaging,the negative group of bone imaging and benign lung disease.Conclusion SPECT imaging in combination with serum CYFR21-1,CEA in the diagnosis of early bone metastasis of lung cancer is more efficient than either single test.The combination of two methods is helpful to staging and test the efficacy.
