急性白血病患者bcl-x基因表达的初步探讨

摘　　要：目的探讨凋亡调控基因bcl-x在急性白血病(AL)患者中的表达状态及其临床意义。方法应用半定量反转录聚合酶链反应技术检测bcl-x(bcl-x_L、bcl-x_s)在44例AL患者AL细胞中mRNA水平的表达。结果抑凋亡基因bcl-x_L在AL初治组中的表达比完全缓解和正常对照组明显增高(P<0.01),同时在复发患者中的平均表达水平是初治患者的1.96倍(P<0.01)。促凋亡基因bcl-x_s在各组间的表达差异无统计学意义(P>0.05)。bcl-x_L基因高度表达(bcl-x_L≥0.50)在复发组的发生率为66.7%,比初治组(20.7%)明显增高(P<0.01)。bcl-x_L基因高度表达患者临床治疗无效率为86.7%,与非高度表达组(19.2%)比较差异有统计学意义(P<0.01)。结论bcl-x_L的高度表达可作为AL患者不良预后的监测指标;bcl-x_L的高表达可以降低AL的化疗敏感性而且是AL复发的高危因素,AL的发病可能与抑凋亡基因bcl-x_L的高表达有关。Objective To investigate the expression of bcl-x genes in bone marrow mononuclearcells (BMMNC)of acute leukemia and its effect on response to chemotherapy.Methods Semi-quantitative reverse transcription polymerase chain reaction (SqRT-PCR) was used to detect the mRNA expression of bcl-x_L and bcl-x_s in BMMNC from 44 patients with acute leukemia (AL).Results The expression of bcl-x_L in BMMNC from 44 patients with AL was higher than that in complete remission patients and normal controls.The expres- sion of bcl-x_L in the relapsed patients increased by 1.96 fold,compared with that in the newly diagnosed pa- tients (P<0.01).However,the different expression of bcl-x_s in two groups has not been observed in this study.Failure of therapy was found in 86.7 % cases with the overexpression of bcl-x_L (bcl-x_L≥0.50).The over- expression rate of bcl-x_L in the relapsed patients was 66.7 % and it significantly higher than that in the newly diagnosed patients (P<0.01).Comparing the treatment failure rate of overexpression(86.7) with non-overex- pression(19.2 %),there was significant difference(P<0.01).Conclusion The overexpression of bcl-x_L which inhibit cell programmed death may contribute to pathogenesis of AL and imply the lower sensitivities of leukemic cells to chemotherapy anti higher risk of relapse,the overexpression of bcl-x_L(bcl-x_L≥0.50) can be as a important criterion in the patients with poor response to chemotherapy.
