弥漫性大B细胞淋巴瘤中YB-1核表达的临床意义

摘　　要：目的探讨 Y-盒结合蛋白(YB-1)在弥漫性大 B 细胞淋巴瘤(DLBCL)中表达及其临床意义。方法采用 Envision 两步法检测50例 DLBCL 和10例反应性淋巴结增生(RLH)患者石蜡标本中YB-1与增生细胞核抗原(PCNA)的表达,分析 YB-1表达与临床参数及 PCNA 表达的关系。结果 DLBCL 组与 RLH 组的 YB-1胞质阳性表达率差异无统计学意义(P>0.05),而 DLBCL 组的 YB-1胞核阳性表达率明显高于 RLH 组(P<0.05);DLBCL 临床Ⅲ～Ⅳ期患者的 YB-1胞核阳性表达率明显高于Ⅰ～Ⅱ期(P<0.05);结外淋巴组织侵犯组的 YB-1胞核阳性表达率明显高于结内病变组(P<0.05);骨髓浸润组的 YB-1胞核阳性表达率显著高于无骨髓浸润组(P<0.05);YB-1的胞核阳性表达与 PCNA积分呈正相关(P<0.05);化疗无效组的 YB-1核阳性表达率及 PCNA 积分均明显高于化疗有效组(P<0.05)。结论 YB-1的核阳性表达可能参与了肿瘤的发生;YB-1核阳性表达与肿瘤的高侵袭力及肿瘤细胞的高增生能力密切相关,提示患者预后不良。Objective To investigate the expression and clinical significance of Y-box binding pro- tein-1(YB-1)in diffuse large B cell lymphoma(DLBCL).Methods The YB-1 and proliferation cell nuclear antigen(PCNA)protein were measured by immunohistochemistry on paraffin-embedded specimens in 50 cases of diffuse large B cell lymphoma and 10 cases of reactive lymphoid hyperplasia(RLH),and the relation be- tween YB-1 expression and clinical parameters or PCNA were analyzed.Results There was no significant difference in YB-1 cytoplasm expression between the DLBCL and the RLH groups(P<0.05).YB-1 nuclear expression in the DLBCL group was significantly higher than that in the RLH group(P<0.05).YB-1 nuclear expression in Ⅲ～Ⅳ clinical stage patients was significantly higher than that in Ⅰ～Ⅱ group(P<0.05);The ex-nodal group had higher YB-1 nuclear expression than the in-nodal group(P<0.05);The frequency of YB- 1 nuclear expression was higher in marrow invasion group than that in non-invaded marrow group(P<0.05); YB-1 nuclear expression was positively correlated with PCNA expression(P<0.05).The frequency of YB-1 nuclear expression(P<0.05)and the PI(P<0.05)in inefficacy group was higher than that in efficiency group. Conclusion YB-1 may be involved in carcinogenesis;its nuclear expression is related to the tumor aggres- siveness and tumor cell proliferation,which indicates the poor outcome.
