乳腺癌超声诊断与雌激素受体表达间的关系

摘　　要：目的探讨雌激素受体(ER)在乳腺浸润性导管癌(IDC)和浸润性小叶癌(ILC)中的表达与超声征象、腋窝淋巴结转移、月经状况之间的关系。方法对118例IDC和ILC患者术前检查的超声征象与ER表达进行对照分析。结果118例IDC和ILC患者中,具有毛刺征、后方声衰减征象之一者,肿块ER阳性率显著高于不具有上述征象的肿块(x^2=8.1840,P<0.01;x^2=15.2018 8,P<0.01);有微钙化组ER阳性率比无微钙化组略高,但差异无统计学意义(P>0.05);而肿块大小、腋窝有无淋巴结转移及绝经前后之间,ER阳性表达差异无统计学意义(x^2=0.3185,P>0.05)。结论乳腺IDC和乳腺ILC二维超声征象可在一定程度上反映ER表达水平。Objective To determine the relationship of ultrasonographic features and the expression of estrogen receptor(ER)in infiltrating duetal carcinoma(IDC)and infiltrating lobular carcinoma(ILC)of breast. Methods 118 patients with breast cancer were evaluated by ultrasonography.Ultra sonographic features in- cluded spiculate margin,posterior shadowing,microcalcifications,axillary lymphnodes metastasis,pre-and post-menopause and size of breast tumor.Scanning was performed before operation.The mastectomy speci- mens were stained with immunohistochemistry,and the expression of ER was measured.Results The expres- sion of ER was significantly correlated with spiculate margin and posterior shadowing in IDC and ILC(P<0.01). There were not statistically difference in microcalcifications,axillary lymphnodes metastasis,pre-and post- menopause and size of breast tumor with ER positive expression(P>0.05).Conclusion The biologic behavior and prognosis of breast IDC and ILC can be assessed according to ultrasonographic features to some extent.
