爆破式超声乳化联合拦截劈核术在硬核白内障摘除术中的应用

摘　　要：目的探讨爆破式超声乳化联合拦截劈核术在硬核白内障摘除术中的应用。方法采用爆破式超声乳化联合拦截劈核术施行自内障手术65例(65跟)。其中Ⅲ级核12眼,Ⅳ级核34眼,Ⅴ级核19眼。结果术后第一天视力≤0．2者3眼(4．6％),0．3-0．5者6眼(9．2％),0．6-0．8者46眼(70．8％),＞0．8者10眼(15．4％)；术后三个月视力≤0．2者3眼(4．6％),0．3-0．5者4眼(6．2％),0．6-0．8者37眼(56．9％),＞0．8者21眼(32．3％)；术后1、3个月平均角膜散光与术前相比较,差异无显著性(P＞0．05)。角膜水肿8眼,后囊膜破裂2眼,少量晶体碎核沉入玻璃体1眼。结论爆破式超声乳化联合拦截劈核术,可使分割较硬的晶体核变得更容易,减少了能量的使用,缩短了超乳时间。该方法安全、有效,适合于硬核白内障手术。Objective To investigate the use of bursting phacoemulsification and stop-and-chop in hard nuclear cataracts. Methods 65 cases (65 eyes) were received bursting phacoemulsification and stop-and-chop,hardness of nucleus were gradeⅢin 12eyes,gradeⅣin 34 eyes,gradeⅤin 19 eyes.Results One day after operative,the visual acuity was≤0.2 in 3 eyes ,0.3～0.5 in 6 eyes (9.2%),0.6～0.8 in 46 eyes (70.8% ),＞0.8 in 10 eyes (15.4%);three months after operative,the visual acuity was≤0.2 in 3 eyes,0.3～0.5 in 4 eyes (6.2%),0.6～0.8 in 37 eyes(56.9%),＞0.8 in 21 eyes (32.3%);there was no significant difference be- tween the mean preoperative astigmatism and astigmatism in postoperative 1 month and 3months (p＞0.05) .Moderate corneal edema take place in 8 eyes,posterior capsule rupture appear in 2 eyes,little lens fragments dropped into the vitreous in 1 eye.Conclusion The hard nucleus is easy to be divided by the bursting phacoemulsification and stop-and-chop,so the power and time of phacoemulsitication is de- creased,the method can be used safely and effectively for hard nucleus cataracts.
