CD_(44V6)在宫颈癌中的表达及其意义

摘　　要：目的　探讨CD4 4V6 在宫颈癌中的表达及其意义。方法　选取我院 1995年 2月～ 1999年 4月 ,从未接受过放射治疗、化学治疗及其他特殊治疗的宫颈癌患者标本 60例 ,其中经手术切除的Ⅰ～Ⅱ期宫颈癌标本 37例 ,Ⅲ～Ⅳ期宫颈癌活检标本 2 3例。同期选取正常宫颈上皮组织标本 10例 ,宫颈上皮内瘤样病变 (CIN)标本 14例。采用免疫组织化学SP法观察各组织标本CD4 4V6 的表达。结果　随着由正常宫颈组织向宫颈上皮内瘤样病变乃到宫颈癌的逐步发展 ,CD4 4V6 的表达逐渐增强 ,其中正常宫颈组织与宫颈癌间存在显著性差异 (p <0 0 5 ) ;其在宫颈癌病理学分级中的阳性表达率亦有显著差异 (p <0 0 5 ) ,癌细胞分化越差 ,CD4 4V6 的阳性表达率越高 ;CD4 4V6 在有淋巴结转移者中的阳性表达率较无淋巴结转移者明显增高 (p <0 0 5 ) ;此外 ,CD4 4V6 的阳性表达率与宫颈癌的临床分期有关 ,Ⅰ～Ⅱ期宫颈癌中CD4 4V6 的阳性表达率低于Ⅲ～Ⅳ期者 ,但两者间无显著性差异 (p >0 0 5 )。结论　CD4 4V6 在宫颈癌的发生、发展及转移过程中发挥着重要作用。Objective To study the expression and its clinical significance of CD 44V6 in cervical carcinoma during its genesis,development and metastasis.Methods 60 samples from patients with cervical carcinoma,who were not treated by radiation,chemotherapy and any other treatment,were harvested from February 1995 to April 1999.Among them,37 samples in stage Ⅰ Ⅱ were gotten by ectomy of cervical carcinoma,23 samples in stage Ⅲ Ⅳwere obtained by biopsy.At the same time,10 samples of normal cervical tissue and 14 samples of cervical intraepithelial neoplasia were harvested.Immunohistochemisty(SP method)was employed to detect the expression CD 44V6 in all of samples.Results The expression of CD 44V6 was gradually strengthened during the tumorigenesis,and there was significant difference among different groups( p <0.05),and there was a relationship between the pathological grades of cervical carcinoma and the positive expression rates of CD 44V6 .The worse the differentiation of cancer cell was,the higher the positive expression rate of CD 44V6 .The positive expression rate of CD 44V6 was obviously lower in the cervicalcar cinoma without lymphatic metastasis than that with lymphatic metastasis( p <0.05).In addition,there was a relationship between the positive expression rate of CD 44V6 and the clinical stage of cervical carcinoma,but there was not any different in positive expression rate of CD 44V6 between stage Ⅰ Ⅱ and stage Ⅲ Ⅳ ( p >0.05).Conclusion CD 44V6 might play an important role in the genesis,development and metastasis of cervical carcinoma.
