组织蛋白酶D和nm23基因蛋白在乳腺癌中的表达及与淋巴结转移的关系

摘  要:目的 探讨组织蛋白酶D和nm23基因蛋白在乳腺癌中的表达、与乳腺癌淋巴结转移的关系以及将之作为判断乳腺癌预后的指标的意义。方法 采用免疫组织化学ABC法对43例乳腺癌中的组织蛋白酶D和nm23基因蛋白的表达进行检测,阳性结果的判定采用半定量积分法。结果 组织蛋白酶D在5例正常乳腺组中不表达,5例乳腺良性病变中少数腺上皮细胞内呈弱阳性表达。但在乳腺癌组织中其表达阳性率为76.2%(33/43)。阳性物质呈粗细不均的颗粒状,主要分布于胞浆中,少数病例阳性物质位于细胞膜。39例乳腺癌组织在肿瘤间质及坏死组织中也可见阳性颗粒状物质分布。43例乳腺癌中25例伴有淋巴结转移,其组织蛋白酶D表达的阳性率为92%(23/25)。无淋巴结转移的18例中,组织蛋白酶D表达的阳性率为55.6%(10/18)。统计分析表明,有淋巴结转移的乳腺癌中组织蛋白酶D的表达明显高于无淋巴结转移的乳腺癌组织(P<0.01)。nm23基因蛋白在5例正常乳腺组织和4例乳腺良性病变组织中呈阳性表达,在43例乳腺癌中nm23基因蛋白表达的阳性率为72.1%(31/43),阳性物质为颗粒状,位于胞浆内。在有淋巴强烈转移的乳腺癌中nm23基因蛋白的阳性率为60%(15/25),无淋巴结转移的乳腺癌中nm23蛋白的阳性率为80.9%(16/18)。To determine the expression of cathepsin D and nm23 protein in the breast cancer tissue and the brelationshipbetween their expression and the metastasis of lymph node from breast cancer. Methods The immunohistochemical ABC method was used to examine the expression of cathepsin D and nm23 protein in 43 cases of breast cancer. The positive results was decided by semi-quantitative integral method. Results The staining of cathepsin D was not found in the five cases of normal breast tissues, only weak staining of some of the mammary gland epithelia cells was observed in five cases of the breast benign lesion. Over-expression of cathepsin D occurred in 33/43(76. 2%) of breast cancer cases. The immunoreactive substances were granular and localized in the cytoplasm. A few of positive substances were also seen on the cell membranes. Distributions of cathepsin D of the tumor stroma and necrotic tissues were observed in 39 breast cancer cases. The metastasis of lymph node, lymph node-positive, was determined in 25 cases of 43 breast cancer cases. The positive rate of cathepsin D was 92%(23/25) in lymph node-positive-breast cancers, but the immunostaining of cathepsin D existed in 10/18 cases (55. 6%) of lymph node-negative breast cancers. The expression of cathepsin D in lymph node-positive breast cancer cases was significantly higher than that of lymph node-negative breast cancer cases (P<0. 01). Expression of nm23 protein was observed in the five cases of nomal breast tissues and four cases of the breast benign lesipn. The immunostaining was granular and distributed in the cytoplasm. The total positive rate was 72. 1%(31/ 43)in breast cancer. The positive rate of nm23 protein was 560% (15/25)in lymph node-positive-breast cancer cases, but the positive rate of cathepsin D was 80. 9% (16/18) in lymph node-negative breast cancers.
