C-erbB-2及P53蛋白在乳腺癌前病变中的表达及意义

摘　　要：目的探讨乳腺癌前病变C-erbB-2及P53蛋白表达对乳腺癌早期诊断的意义。方法应用免疫组化S-P法检测C-erbB-2及P53蛋白在不同程度乳腺导管上皮增生42例、乳头状瘤病86例及导管内癌28例、浸润性癌20例中的表达。如果C-erbB-2在轻度和重度导管上皮不典型增生及中、重度现头状瘤病均出现阳性或／和强附性表达.阳性串分别为五82％（2／11）、388％（7／18）和25.0％（7／28）、32.4％（12／37）.至导管内画表达阳性率达高峰,为50.0％（14／28）,而至浸润性癌阳性率反而降至35.0％（7／20）.P53蛋白在导管上皮增生及孔头状瘤病均无阳性表达,至导管内癌出现少许阳性表达,阳性牢3.5％（1／28）。而至浸润性癌达高峰,阳性率为25.0％（5／20）。结论C-erbB-2可用于筛选早期可疑乳腺癌,而P53蛋白则无早期诊断价值.Objective To evaluate the levels of C-erbB-2 and P53 protein in precancerous lesions of breast and it's value for early diagnosis of breast cancer. Materials and Methods Levels of C-erbB-2 and P53 protein were detected by immunohistochemistry in ductal hyperplasia 42 cases,papillomatosis 86 cases and intraductal carcinoma 28 cases,infiltrating carcinoma 20 cases. Results CerbB-2 protein expression was seen in mild,high atypical hyperplasia and moderate,high proliferated papillomatosis. The positive rates of them were 18. 2%, 38. 8% and 25. 0%, 32. 4%. The rate of overexpression increased with the degree of proliferation. The P53 protein expression was not found in atypical hyperplasia and papillomatosis. Conclusion C-erbB-2 assay may be helpful for detection of early malignant change of atypical hyperplasia and high proliferated papillomatosis. P53 protein is of no significant diagnostic value in these lesions.
