摘  要:多药耐药(MDR)是急性白血病化疗成功的主要障碍之一,其机理涉及诸多因素。建立能够体外准确预测化疗药物敏感性试验方法,在指导急性白血病治疗中具有重要意义。本文研究了42例急性白血病患者体外药敏检测(MTT法)和白血病细胞P-gp)抗原的表达与白血病化疗疗效的关系。结果显示:药敏试验与治疗疗效有相关性,临床总符合率82.9%,阳性符合率78.9%,阴性符合率86.9%。认为P-gp的表达是一个有效的化疗耐药指标,白血病缓解病人P-gp阳性率高于初诊病人,P-gp阳性表达的患者完全缓解率明显低于P-gp阴性患者。Multidrug resistance (MDR) remains a major obstacle to successful treatment of acute leukemia and its mechanism involves multiple factors. It would be of great importance to establish a panel of in vitro predictive tests to conduct clinical practice in the treatment of acute leukemia. The present study investigated the correlation between chemosensitivity assay in vitro (MTT method) and P-gp pheno-type expression of leukemic cells and the outcome of chemotherapy in 42 patients with acute leukemia. The results of the study revealed that the chemosensitivity assay correlated well with the outcome of the treatment with an overall predictive rate of 82.9%, a positive predictive rate of 78. 9% and a negative predictive rate of 86.9%, respectively. P-gp expression was found to be an useful parameter to predict the outcome of the treatment. Patients with relapsed disease had higher rate of positivity for P-gp expression than that in newly diagnosed patients. Patients with positive expression of P-gp showed a significant lower rate of complete remission than patients with negative for P-gp.
