双侧同期功能性颈淋巴清扫术在口腔颌面部癌瘤治疗中的初步探讨

摘　　要：本文报告6例双侧同期功能性颈淋巴清扫术。全部为T_3—T_4病例。术中保留双侧颈内静脉、付神经及胸锁乳突肌。术后无颜面水肿、紫绀、咽喉水肿、球结膜充血、眼球突出等症状,更无手术死亡病例。采用本术式不用低温麻醉、蛛网膜下腔留置导管、头位抬高、大剂量激素应用、控制补液量、气管切开及保留颈外静脉等确保安全的必要措施,避免了双侧同期根治性颈清扫术因切除双侧颈内静脉引起颅内压增高的主要危险。In this paper 6 cases of bilateral simultaneous funcational neck dissection are reported, T3-T4 cases altogether.The bilateral internal jugular veins, sternocleidornastoideus, aceessorius nervus are preserved in operation. No facial edema and eyonosis, Laryngeal and pharyngeal edema, chemosis exopthalmos were found after operation, and also none of the postoperative death occured. This method demonstrates that measures for assuring the securing of the operation,such as hypothermal (?)nesthesia, retaining cathetec in the intra-arachnoid space, elevated position of the head, use of a great quantity hormone(dexamethasone), control of the intravenous infusion, tracheotomy, preser-vating the external jugular vein et al, are unnecessary. It is completely avoided the main danger of elevation of intracrainal pressure after dissection Of bilateral internal jugular vein in cases of BSRND.
