乳房下切口胸骨正中劈开法行心内直视手术4例

摘　　要：以往常用的胸骨正中劈开途径的优点是不切断胸壁肌群,不打开胸腔即可直至心脏进行直视手术,出血较少,但术后在胸前遗留较大的切口瘢痕。这一点难以被女性病人(尤其是年轻女性)所接受。我们根据Brom所提出的沿乳房下横切开皮肤加胸骨正中劈开法,对4例先天性心脏病病人施行心内直视手术,从而避免了在胸前遗留较大切口瘢痕。现介绍具体手术方法及其结果。病例摘要病例1:女,16岁,诊断为先天性室间隔缺损。1987年7月9日在体外循环下施行心内直视室间隔缺损修补术。病例2:女,12岁。One patient with atrial defect and three patients with ventricular septal defect were operated on with Brom's submammary skin incision for median sternotomy in intracardiac repair under direct vision. The operative process was carried out successfully, the operative scar has healed well. Compared with the traditional incision for median sternotomy, the time of the open-heart by-pass surgery is not any longer than usual. The operative scar is totally concealed under a brassiere after the operation on female patients. Considered cosmetically, this operative method suits female patients, especially young female patients, and it deserves recommendation, in clinical work.
