慢性胃溃疡癌变的初步探讨——附54例良、恶性胃溃疡临床病理分析

摘　　要：作者对54例良、恶性胃溃疡手术切除标本进行病理组织学比较观察。发现其中3例恶性胃溃疡与溃疡型胃癌有明显不同,而在溃疡组织结构上与慢性胃溃疡很相似,且癌多以早期的形式存在于溃疡边缘,溃疡底部存在一个无癌浸润、无肌残留的疤痕区。这类溃疡的存在从组织学上支持了慢性胃溃疡癌变的事实,但本组资料的癌变率较低,约为0.52％左右,提示慢性胃溃疡癌变在胃癌发病学上意义不大。A series of 54 cases of resected specimens with benign and malignant gastric ulcers was collected. The comparative observations of the base and the margin of both ulcers were performed.It was found that the malignant ulcers existed in the form of a special gastric ulcer-chronic gastric ulcer(CGU),in the margin of which early carcinoma infiltration appeared and in the certer of whose base no carcinomatous infiltration and muscle residue were present. This malignant gastric ulcer was different from cancerous ulcer. The results support the fact that CGU has a cancerous change. In our material, the rate of cancerous change is only 0.52%. It reveals that GGU is of little significance for predicting its convertion into gastric carcinoma.
