体外震波粉碎胆囊结石的实验研究及临床应用

摘  要:我们运用国产 NE—Ⅱ型体外震波胆碎石机进行了体外碎石试验、动物胆囊结石碎石试验及治疗28例胆囊结石、2例胆总管结石病人。实验研究表明:该机在体外以30~50J、100~640震次,在体内以40~50J,618~2000震次可将胆石粉碎至2~3mm以下或胆泥,且对组织脏器无严重损伤。临床治疗30例,其中1例未碎,碎石率为96.43%。震后1个月随访15例,B 超证实5例结石完全消失(4例胆囊结石、1例胆总管结石)。除1例胆总管结石病人震后1周内有黄痘、腹痛外,其他病人无任何特殊不适。震波碎石前后血清酶、摄胸片等检查无异常改变。We perfomed experiments of disintegrating biliary stones both in vitro and in vivo, and first treated 28 patients with gallbladder calculi and 2 patients with common bile duct stones using Chinese NE-I model extracorporeal shock wave biliary lithotripter. studies showed that the biliary stones could be disintegrated into sludge or fregments less than 2~3mm in diameter within a range of 30 to 50 J,100 to 640 shock wave discharges in vitro and a range of 40 to 50 J,618 to 2000 discharges in vivo,and shock-wave-induced severe damages of tissues in animal were not found.The biliary stones were disintegrated in all but one patients and the rate of disintegration was 96.43%.The biliary stones failed to be disintegrated in 3 of 15 patients within one month after lithotripsy(2 with gallbladder calculi,1 with common bile duct stone).All patients had no discomfort during and after lithotripsy except jaundice and upper abdominal pain in a patient with common bile duct stone within one week after lithotripsy.Serum enzymology and chest X-ray examination in all patients did not show abnormality after lithotripsy.
