乳腺某些良恶性疾病CEA免疫组织化学的定位研究

摘　　要：本实验研究用 PAP 免疫组织化学方法观察了84例乳腺癌和24例乳腺良性病变组织石蜡切片中 CEA 的存在情况及其分布形式。结果表明:乳腺癌69例为阳性,阳性率为82.1%,导管浸润癌比特殊类型癌 CEA 阳性率为高(P<0.01).CEA 的分布呈两种形式,一种为胞质为主型,多见于导管特殊类型癌,另一种为胞膜为主型,多见于特殊类型癌。8例伴有导管上皮增生的乳腺结构不良症,增生性导管上皮全部为 CEA 阳性,其余良性乳腺疾病均为 CEA阴性。乳腺癌 CEA 阳性与患者年龄、间质淋巴细胞浸润、组织学分级以及组织在蜡块中的保存年限(26年内)无明显关系.乳腺癌间质弹力纤维增生组比无弹力纤维增生组 CEA 阳性率为高(P<0.01),但与弹力纤维增生程度无关。The peroxidase antiperoxidase (PAP) technique was used to localize CEA in 84 cases of primary breast carcinoma and 24 cases of benign mammary lesions.Incidence of positvity for CEA in breast carcinomas was 82.1%.Rate of CEA positivity in infil- trating ductal carcinoma was higher than special histologic variants of carcinoma (P<0.01).Distribution of CEA in breast carcinomas has two types.Most of the membranous types of antigen were found in the special histologic variants of carcinoma, cytoplasmic type was often found in the infiltrating ductal carcinoma.Eight cases of mammary dysplasia with ductal epithelial hyperplasia were positive for CEA,other cases of benign mammary lesions were negative.There is no significant relationship bwtween CEA positivity in breast carcinoma and age,lymphocytic infiltrate,histologic grade,time of par- affin-embeded tissue (in 26years).Rate of CEA positivity in the group of stromal elastosis in breast carcinoma was higher than the group of no stroma] elastosis (P<0.01) ,but there is no relation in our study between CEA positivity and degree of stromal elastosis in breast carcinoma.
