不同粘结剂等因素对F_(2000)复合体修复楔状缺损影响的临床观察

摘　　要：目的:用改良USPHS系统评价2种粘结剂及相关因素对F2000复合体修复楔状缺损临床疗效的影响。方法:69例患者417颗楔状缺损患牙均用F2000复合体修复,其中A组207颗牙用全酸蚀粘结体系(Sconchbond Multi-Purporse酸蚀剂,Adper Single Bond粘结剂),B组210颗牙用AdperPromptL-Pop自酸蚀粘结剂。修复后1w、1、6、12、24个月用改良USPHS系统评价其疗效,并对其相关影响因素进行初步探讨。结果:F2000复合体修复楔缺12、24个月后总成功率分别94.32%、87.33%。其中A组成功率分别为95.09%、88.36%,与B组(93.53%,86.36%)比较无统计学差异。但按USPHS系统逐条分析,A组的边缘不密合和边缘着色发生率为1.06%、0.53%,低于B组(4.55%,3.53%),有统计学差异。其疗效还受咬合力、楔缺程度、刷牙方法等多种因素影响。结论:适当的充填材料和酸蚀粘结体系、调牙合、消除致病因素及必要的宣教,均是修复楔缺成功的重要因素。Objective： To evaluate the curative effects of different adhesive agents and relative factors for the restoration of dental wedge-shaped defects with F2000 Compomer by modified USPHS criterion. Methods： 417 teeth with wedge-shaped defects selected from 69 cases were divided randomly into two groups which include 207 and 210 teeth respectively, and were restored by F2000 compomer. The A group were bonded with total-etching adhesive （Sconchbond Multi-Purporse, Adper Single Bond adhesive）, the B group with all-in-one self-etching adhesive （Adper Prompt L-Pop）. After 1 week and 1,6,12,24 months, followed-up results were evaluated with modified USPHS evaluation criterion and its influence factors were studied. Results： After 12 and 24 months, the total successful rates of all the teeth were 94.32%, 87.33%, 95.09% and 88.36% in groupA, and 93.53%, 86.36% in group B respectively, the difference are not significant statistically. But there were significant differences in marginal integrity and marginal tain rate between the two groups by USPHS evaluation criterion,. Furthermore, the successful rates were related with occlusal force, the abrasive degree of the restored teeth, tooth brushing, etc. Conclusions： Appropriate filling material and acid etching adhesive, occlusal adjustment, eliminating nosogenesis and necessary instruction are all important for successful treatment of wedge-shaped defect.
