摘  要:目的研究超声光散射成像系统测量的最大血红蛋白浓度值(maximum hemoglobin concentration,MHC)随肿瘤内实际血红蛋白(Hb)浓度变化规律,以及肿瘤大小和深度对MHC值测定的影响。方法将新鲜抗凝人血用0.6%的脂肪乳盐水溶液稀释成不同Hb浓度的红细胞悬液,灌人硅胶囊制成不同直径和不同Hb浓度球囊,将球囊固定在盛有0.6%脂肪乳溶液的容器中。应用超声光散射乳腺成像系统分别测定不同浓度、直径和深度球囊的MHC值,并与实际Hb浓度对比分析。结果(1)在球囊直径和深度固定的情况下,MHC与球囊内实际Hb浓度间呈自然对数关系[MHC-37.5×In(实际Hb浓度)-69.7];(2)在球囊内实际Hb浓度和深度固定的情况下,MHC随球囊直径的增大而增大(MHC-10.9×直径~43.9);(3)在球囊内实际Hb浓度和直径固定的情况下,MHC随球囊深度的增加而减小(MHC-二-7.7×深度+268.4)。结论MHC能在一定程度上反映肿瘤内实际Hb浓度,肿瘤的大小和深度显著影响MHC的测量。Objective To study the regularity between maximum hemoglobin concentration (MHC) and the real hemoglobin concentration in breast tumor, and analyze the effect of tumor size and depth on MHC detected by ultra sound-diffuse optical imaging system. Methods Used 0.6% lipid emulsion to dilute human anticoagulation blood to form different hemoglobin concentration suspensions of red cell, then poured these suspensions into silica gel sacculi to make sacculi of different diameter, then fixed these sacculi in one of two containers filled with 0.6% lipid emulsion, respectively. When the hemoglobin concentration, the sacculus diameter or the depth was changed, MHC was compared with the real hemoglobin concentration in the sacculus. Results (1) if sacculus diameter and depth were fixed, the relationship of natural logarithm existed between MHC and the real Hb concentration in the sacculus EMHC= 37.5Xln (the real Hb concentration)-69.7]; (2) if the real Hb concentration and sacculus depth were not changed, MHC increased with the accretion of sacculus diameter [MHC = 10.9 X diameter--43.9];(3) if the real Hb concentration and sacculus diameter were not changed, MHC decreased with the augment of saceulus depth [MHC = -- 7.7 X depth 27 268.4]. Conclusions MHC detected by US-guided optical imaging system can approxi- mately reflect the real hemoglobin concentration in breast tumor, the size and depth of breast tumor significantly influence the detection of MHC.
