血清CA125诊断肝癌的价值

摘　　要：目的研究血清ＣＡ１２５对肝癌的诊断价值．方法肝癌患者３２例，男２０例，女１２例，平均年龄４６岁±２５岁；肝硬变３６例，男２７例，女９例，平均年龄４２岁±２３岁；正常对照组４０例．全部受测对象均静脉采血，分离血清，－２０℃贮存备测．采用ＩＲＭＡ法测定血清ＣＡ１２５含量．结果肝癌患者血清ＣＡ１２５含量２１８Ｕ／Ｌ±１３０Ｕ／Ｌ，显著高于肝硬变组和正常对照组（分别为１９Ｕ／Ｌ±１１Ｕ／Ｌ，１０Ｕ／Ｌ±８Ｕ／Ｌ，Ｐ＜００１）．肝硬变组高于正常对照（Ｐ＜００５），但显著低于肝癌组（Ｐ＜００１）．以２６Ｕ／Ｌ为界限值，肝癌阳性检出率为６８７％，其中ＡＦＰ阳性（ＡＦＰ＞４００μｇ／Ｌ）的肝癌中血清ＣＡ１２５的阳性检出率为７６０％，ＡＦＰ阴性（ＡＦＰ＜２０μｇ／Ｌ）的肝癌中血清ＣＡ１２５的阳性检出率为４２９％．AIM To study the diagnostic value of serum carbohydrate antigen 125 (CA125) in liver carcinoma. METHODS Thirty two patients with liver carcinoma (male 20, female 12, average age 46±25 years), 36 cirrhosis (male 27, female 9, average age 42±23 years) and 40 healthy persons as controls were studied. Blood samples were taken by venipuncture. Serum was separated and frozen at -20℃ until the analysis was made. Serum CA125 was measured by IRMA. RESULTS Serum CA125 in liver cancer (218U/L±130U/L) was significantly higher than that in cirrhosis and normal controls (19U/L±11U/L and 10U/L±8U/L, P <0 01). In cirrhosis group it was higher than that of normal controls ( P <0 05), but was markedly lower than in liver carcinoma group ( P <0 01). When 26U/L was taken as the cutoff value, the positive rate in liver cancer group was 68 7%. The positive rate of serum CA125 in liver carcinoma with positive AFP (AFP>400μg/L) was 76 0% and that with negative AFP (AFP<20μg/L) was 42 9%. CONCLUSION CA125 is a better tumor marker in the diagnosis of liver carcinoma.
