果糖及木糖醇注射液对老年2型糖尿病患者血糖波动的影响

摘　　要：目的应用动态血糖监测系统(CGMS)评估果糖及木糖醇注射液对老年2型糖尿病血糖波动的影响,为老年2型糖尿病患者的补液治疗提供选择依据。方法选择稳定期老年2型糖尿病患者20例,采用自身对照方法,随机在第1,2,3天9:00～11:00分别匀速静脉滴注0.9%生理盐水200ml、5%果糖注射液200ml或5%木糖醇注射液200ml。同时用CGMS监测血糖的动态变化,比较9:00～12:00血糖波动情况。结果生理盐水组、果糖组、木糖醇组9:00～12:00(180min)内血糖最高值〔(10.63±2.42)vs(9.87±1.96)vs(10.18±1.44)mmol/L〕、血糖最低值〔(5.34±1.72)vs(5.95±1.68)vs(5.63±1.45)mmol/L〕、血糖平均值〔(7.78±1.85)vs(7.73±1.42)vs(7.19±0.87)mmol/L〕、血糖波动最大幅度〔(5.29±2.24)vs(3.93±2.53)vs(4.56±2.15)mmol/L〕、血糖标准差〔(1.67±0.61)vs(1.49±0.65)vs(1.62±0.61)mmol/L〕及血糖曲线下面积(AUC)〔(1377.40±295.51)vs(1391.75±258.57)vs(1425.84±157.38)mmol.min-1.L-1〕。三组间血糖最高值、最低值、平均值、血糖波动最大幅度、血糖标准差及血糖AUC均无显著区别(P>0.05)。结论果糖注射液及木糖醇注射液对老年2型糖尿病患者的血糖波动无明显影响,可作为其补液治疗的安全选择。Objective To investigate the effect of fructose and xylitol injection on blood glucose fluctuation of elderly type 2 diabetes（T2DM）patients by continuous glucose monitoring system（CGMS）and provide them with evidence for the selection of rehydration treatment.MethodsBy self-controlled methods,20 elderly T2DM patients in stable phase were selected and were injected with 0.9% normal saline 200 ml,5% fructose injection 200 ml or 5% xylitol 200 ml on 9：00～11：00 from the 1st day to the third day randomly at constant speed.The blood glucose dynamic variation was monitored by CGMS,and the fluctuation of blood glucose from 9：00 to 12：00 was compared.ResultsThe maximum（Max）,minimum（Min）and mean（Mean）of blood glucose,Max amplitude of blood glucose fluctuation（MAGE）,standard deviation of blood glucose（SD）and area under curve（AUC）of patients from normal salines,fructose and xylitol groups on 9：00～12：00 were as the followings：Max〔（10.63±2.42）vs（9.87±1.96）vs（10.18±1.44）mmol/L〕,Min〔（5.34±1.72）vs（5.95±1.68）vs（5.63±1.45）mmol/L〕,mean〔（7.78±1.85）vs（7.73±1.42）vs（7.19±0.87）mmol/L〕,MAGE〔（5.29±2.24）vs（3.93±2.53）vs（4.56±2.15）mmol/L〕,SD〔（1.67±0.61）vs（1.49±0.65）vs（1.62±0.61）〕,AUC〔（1 377.40±295.51）vs（1 391.75±258.57）vs（1 425.84±157.38）mmol·min-1·L-1〕correspondingly.No significant differences of Max,Min,Mean,MAGE,SD or AUC were found among patients from three groups（P〉0.05）.In addition,no adverse effects were reported.ConclusionsFructose and xylitol injections have little effect on blood fluctuation of T2DM patients and can be chosen for rehydration safely.
