摘 要:机械通气是呼吸衰竭患者重要的支持手段,但传统的机械通气存在明显人机不同步,可能导致机械通气时间延长、呼吸机相关肺损伤等相关并发症。神经电活动辅助通气,通过监测膈肌电活动,根据自身吸气驱动,成比例地持续辅助通气。目前研究表明神经电活动辅助通气明显改善有创及无创机械通气人机同步性;减轻呼吸肌肉负载;能自动调节通气支持水平,具有一定的肺保护作用;利用膈肌电活动,发挥独特的监测功能。Mechanical ventilation is a life-saving intervention for the management of acute respiratory failure. Patient-ventilator asynehrony may develop in controlled or assisted modes of conventional mechanical ventilation,which will result in a series of adverse effects, such as prolonged mechanical ventilation and ventilator-induced lung injury, et al. Neurally adjusted ventilatory assist (NAVA) is a new mode of mechanical ventilation delivering continuously ventilatory assist in proportion to the patient's respiratory drive measured by the electrical activity of the diaphragm (EAdi). There are some reports resulting from studies on NAVA which demonstrate that NAVA improves patient-ventilator synchrony in invasive and non- invasive ventilation, unloads respiratory muscle. Moreover,NAVA can adjust the level of assistance that can be considered lung-protective in response to changes in neural drive automatically. Besides the use of EAdi to control the respirator,NAVA possesses unique monitoring capabilities with monitoring the EAdi signal.
