分泌型磷脂酶A2在胃腺癌组织中的表达意义

摘　　要：目的探讨分泌型磷脂酶A2（sPLA2）在胃腺癌组织中的表达意义。方法收集2006年1月至2007年4月吉林大学第二医院胃腺癌手术标本65例，正常胃黏膜组织标本11例。65例胃腺癌中，高分化11例、中分化10例、低分化44例，其中淋巴结转移50例。检测sPLA2在胃腺癌及正常胃黏膜组织中的表达情况，分析sPLA2与胃腺癌的分化程度、淋巴结转移、幽门螺杆菌（Hp）感染的关系。率的比较采用Χ^2检验，关联性采用Spearman轶相关分析。结果sPLA2在正常胃黏膜及胃腺癌中的阳性表达率分别为36％（4／11）、78％（51／65）。低、中、高分化胃腺癌sPLA2阳性率分别为84％（37／44）、70％（7／10）、64％（7／11）。sPLA2表达程度与肿瘤的恶性程度呈正相关（r=0．272，P〈0．05）。淋巴结转移阴性和阳性的胃腺癌sPLA2表达率分别为47％（7／15）、88％（44／50）.sPLA2表达与淋巴结转移有关（Χ^2=9．347，P〈0．05）。Hp感染阴性和阳性的胃腺癌sPLA2表达率分别为76％（13／17）、79％（38／48）。sPLA2的表达与Hp感染无关（Χ^2=0．000，P〉0．05）。结论sPLA2基因激活可能与胃癌发生有关；sPLA2可作为一个影响胃腺癌浸润行为和预测不良预后的指标。Objective To investigate the expression and signficance of secretory phospholipase A2 （sPLA2） in gastric adenoearcinoma. Metbods Sixty-five samples of gastric adcnoearcinoma （ 11 with high differentiation, 10 with median differentiation and 44 with low differentiation） and 11 samples of normal gastric mueosa had been obtained from the Second Affiliated Hospital of Jilin University from January 2006 to April 2007. Fifty samples of gastric adenocareinoma were found with lymph node metastasis. The expression of sPLA2 in gastric adenoearcinoma and normal gastric mucosa was detected. The relationship between sPLA2 and the differentiation of gastric adenocareinoma, lymph node metastasis and helicobaeter pylori iniection was detected. All the data were processed with ehi-square test or Spearman rank correlation. Results The positive rates of the sPLA2 expression in normal gastric mucosa and gastric adenocarcinoma were 36% （4/11 ） and 78% （51/65）. The positive rates of sPLA2 expression in low, median and high differentiated gastric adcnocarcinoma were 84% （37/44）, 70% （7/10） and 64% （7/11）, respectively. The expression of sPLA2 was positively correlated with the malignancy of gastric adenoearcinoma （ r = 0.272, P 〈 0.05）. The positive rates of sPLA2 expression in gastric adenocareinoma with and without lymph node metastasis were 88% （44/50） and 47% （7/15）, respectively. The expression of sPLA2 was correlated with lymph node metastasis （ Χ^2 = 9. 347, P 〈 0.05 ）. The positive rates of sPLA2 expression in gastric adenocareinoma with and without helicobacter pylori infection were 79% （38/48） and 76% （ 13/17）, respectively. The expression of sPLA2 was not correlated with helicobacter pylori infection （ Χ^2 = 0. 000, P 〉 0. 05 ）. Conclusions The activation of sPLA2 gene may be correlated with the genesis of gastric adenoearcinoma. sPLA2 may influence the invasion of gastric adenocarcinoma, and can be used as an indicator in predicting poor prognosis.
