摘　　要：目的:探讨老年人轻度认知功能障碍患者海马体积的变化特点及其与简易智力状态检查的相关性。方法:对照组14例,轻度认知功能障碍组16例,由主治医师用简易智力状态量表(MMSE)对具有轻度认知功能障碍的老年人进行筛查:完成ADL总体衰退量表和Hachinski缺血指数评定排除血管性痴呆,SAS及SDS,HAMA及HAMD排除焦虑及抑郁;对MCI患者及正常老年人进行MRI检查。采用西门子Trio-Ti 3.0T MRI成像系统,运用IBASPM软件系统,用MRI三维测量法及简易智力状态检查,比较2组海马体积变化及其与简易智力状态检查评分的相关性。结果:老年轻度认知功能障碍组左侧海马体积、右侧海马体积及总海马体积明显低于对照组(P<0.05)。偏相关分析表明3组海马体积与简易智力状态检查评分呈显著的正相关(r=0.812,P<0.01;r=0.839,P<0.01;r=0.852,P<0.01)。结论:海马体积萎缩的MRI定量测定有助于轻度认知功能障碍的诊断;海马体积萎缩是老年人认知功能障碍的重要影响因素之一。Objective To evaluate the correlation between the changes of hippocampal volume and MMSE score in elder patients with mild cognitive impairment. Methods Sixteen senile patients and 14 controls were evaluated with the MMSE, global deterioration scale and Hachinski ischemic score. Screening of mild cognitive impairment patients was accomplished by the trained investigators in research team according to the grouping condition mentioned above. Three-dimensional measurement of hippocampal volume was performed by MRI tissue segmentation and volume measurement technology. Difference of hippocampal volume was compared by multivariable covariance analysis in order to exclude the influence of age and sexuality. Partial correlation analysis was used in study of relation between hippocampal volume and MMSE score. Results By controlling the effect of age and sex, the left side, right side and total hippocampal volume were smaller in the patients with mild cognitive impairment than those in controls （P〈0. 05）, and they were positive correlated with the change rate of MMSE score （r=0. 812, P〈0. 01; r=0. 839, P〈0. 01; r=0. 852, P〈0. 01）. Conclusion MRI quantitative estimation of hippocampal atrophia can help the diagnosis of mild cognitive impairment. Hippocampal volume atrophia is an important influential factor of cognitive impairment in senile people.
