醒脑静注射液对脑出血术后病人脑代谢的影响

摘　　要：目的:探讨醒脑静注射液对脑出血术后患者脑代谢的影响。方法:选择40例脑出血术后患者,随机分为治疗组和对照组,行桡动脉和颈内静脉血气分析及血糖、血乳酸、血红蛋白监测,计算动-颈内静脉血糖差、颈内静脉-动脉乳酸差以及脑氧摄取率,观察其动态变化及两组间的差别。结果:治疗前两组数据相比无明显差异(P>0.05),在治疗第1天,两组动-颈内静脉血糖差及颈内静脉-动脉乳酸差无差别(P>0.05),治疗组脑氧摄取率低于对照组(P<0.05);在治疗第3、5、7天,治疗组动-颈内静脉血糖差及脑氧摄取率高于对照组(P<0.05),治疗组颈内静脉-动脉乳酸差低于对照组(P<0.05或P<0.01)。结论:醒脑静注射液可以明显改善脑出血术后患者脑水肿期的脑氧摄取率及动-颈内静脉血糖差,降低颈内静脉-动脉乳酸差。Objective ：To evaluate the effects of Xinguaojing Injection on postoperative cerebral metabolism in patients with cerebral hemorrhage. Methods：Forty patients were divided into two groups randomly, therapy group and comparison group. Monitor dynamically blood gas analysis, glucose ,lactic acid ,hemoglobin in radial artery and internal jugular venous bulb, accounting the cerebral oxygen extraction（ CEO2 ）and the D -values of glucose and lactic acid respectively between radial artery and internal jugular venous bulb. Results：Data have no difference between the two group before therapy（ P 〉0. 05）. The D -values of glucose and lactic acid have no difference between the two group on first day after therapy,the CEO2 of therapy group was less than that of comparison group on first day after therapy（ P 〈0.05 ）. The CEO2 of therapy group was more than that of comparison group on third ,3rd,5th,7th day after therapy （P 〈 0. 05 ）. The D -values of glucose in therapy group were greater than comparison group on third,3rd,5th,7th day after therapy, but that of lactic acid were less than the other group（P 〈0. 05 or P 〈0. 01 ）. Conclusion：Xingnaojing Injection can improve postoperative CEO2 and the D - values of glucose, decreasing I） - values of lactic acid, between radial artery and internal jugular venous bulb in patients with cerebral hemorrhage （edema phase ）evidently.
