15例婴儿痉挛患儿与24h脑电图特点及临床分析

摘　　要：目的探讨15例婴儿痉挛患儿临床与24h脑电图异常的特点。方法2005年12月-2007年12月在我院儿科门诊或病房诊断的15例婴儿痉挛患儿进行24h脑电活动回顾性分析。结果24h脑电图显示:典型高度失律6例,不对称高度失律2例,局灶高度失律2例,周期性高度失律5例,2例无典型高度失律,其中1例局灶性痫性放电,1例背景活动变慢。15例中有2例24h脑电活动中除典型高度失律,还可见到周期性高度失律存在。治疗后脑电图高度失律消失,7例示低电压背景活动,2例脑电图上仍存在背景活动异常,1例出现弥漫性1.5-2 HZ慢棘慢复合波,3例局限性尖波和棘波发放。结论婴儿痉挛患儿24h脑电图在发作间期主要脑电图表现为高度失律,高度失律有多种形式,一次记录中,典型高度失律和其他变异型高度失律可以存在于同一患儿,我们应提高认识。治疗后脑电图高度失律消失但仍各有不同,其临床意义有待我们进一步去揭示。Objective： To explore the clinical and 24 -hour active -electroencephalogram （AEEG） features of fifteen patients of infantile spasms. Methods ： We reviewed the 24 - hour active - electroencephalographic of 15 cases of infantile spasms from our pediatric admission Dept （department） and out- patient dept from, December 2005 to December2007. Results： 24 -hour AEEG appearances： 6 cases showed typical hypsarrhythemia; 2 cases showed asymmetrical hypsarrythemia; 2 cases showed focal hypsarrhythemia; 5cases showed periodic hypsarrythemia 2 cases showed no hypsarrhythemia 2 cases of all showed both typical hypsarrhythemia and periodic hypsarrythemia. After therapy , all patients＇hypsarrhythemia disappeared, 7 cases showed low -voltage background activity. 2 eases showed abnormal background activity. 1 cases showed diffused spike - slow complex wave of 1.5 - 2 HZ. 3 cases showed focus wave of sharp and spiike. Conclusion： 24 - hour AEEG of infantile spasms in interval seizures suggests that there are varied hypsarrythemia forms, and these varied hypsarrythemia forms may show in the same patient＇s record, we should recognize them. After therapy, all patients＇ hypsarrhythemia disappeared, although varied abnormal EEG existed, and we should to reveal its clinical siznificance.
