摘  要:目的探讨颌骨原发性骨肿瘤和肿瘤样病变的影像学表现,提高其诊断水平。方法回顾性分析经病理证实的颌骨骨肿瘤及肿瘤样病变94例。结果94例中,颌骨良性肿瘤及肿瘤样病变87例,其中牙源性64例,占全部病例的68.09%;非牙源性23例,占全部病例的24.47%,其CT表现为边缘清晰,无钙化、瘤骨及骨膜反应。13例成釉细胞瘤则表现为边缘规则的多房或单房肿块,膨胀性骨质破坏。恶性颌骨肿瘤7例,占全部病例的7.44%,CT主要表现为不规则软组织肿块,呈溶骨性骨质破坏,肿块内有瘤骨形成等征象。CT在显示病变边缘、病变与骨质和周围软组织的关系上较X线优越。结论颌骨原发性骨肿瘤和肿瘤样病变种类繁多,良性牙源性肿瘤居多,影像学检查以CT为首选,诊断需结合临床和病理。Objective To study the imaging appearances of primary bone tumor and tumor-like lesions of the jaw bones,in order to improve the diagnosis.Methods 94 cases with primary bone tumor and tumor-like lesions of the jaw bones were confirmed by pathology.Their medical imaging appearances were reviewed.Results In 94 cases,68.09%(64/94 cases)had odontogenic tumor,24.47%(23/94 cases)had non-odontogenic tumor.CT showed masses with regular margin,without calcification,new tumor bone formation or periosteal reaction.13 cases of ameloblastoma showed multilocular bumps with regular margins and bone expansion.7.44%(7/94)had the malign bone tumor of jaw bones.CT showed irregular soft-tissue mass with osteolytic bone destruction,and new tumor bone formation.CT was superior to X-ray in the evaluation of lesion margin and its relationship with cortical bone and soft tissues.Conclusion The primary bone tumor and tumor-like lesions of the jaw bones are various,and the majority is benign odontogenic tumor.CT should be the first choice in diagnosing primary bone tumor and tumor-like lesions of the jaw bones.Colligation of the clinical and histological data is helpful for diagnosis.
