摘  要:目的分析颅底脊索瘤的MRI特征,评价肿瘤T2WI信号强度与病理分型间的关系。方法回顾性分析16例经病理证实的颅底脊索瘤的MRI资料,包括部位、生长方式、信号特点、强化方式及与邻近结构的关系;评价肿瘤T2WI信号强度,并将其与病理分型进行对照分析。结果16例中12例发生于斜坡,4例位于斜坡以外,其中鞍旁、颈静脉孔区各1例,蝶窦2例。病变均边界清楚、形态规则或不规则(分叶状)。病变信号不均,均可见蜂窝状表现。T1WI呈明显低或稍低信号,其中8例可见局灶性高信号;T2WI均为高或稍高信号。增强后均显示不均匀强化,其中轻度强化4例,中度强化6例,缓慢渐进性强化6例。多方位成像可以清晰显示病变位置及与邻近重要结构的关系。16例中10例为经典型,6例为软骨瘤样型。两型之间T2WI的信号强度差异无统计学意义。结论颅底脊索瘤MRI无论在发病部位、信号特点及强化方式上都具有一定的特征性表现;单纯根据病变的信号强度无法确定脊索瘤病理分型。Objective To analyze the MR features of chordoma in the skull base, and to explore the relationship of T2WI signal intensities and pathologic types of chordoma. Methods MR images were retrospectively analyzed in 16 patients with pathologically confirmed chordoma in the skull base. MR features were evaluated, including tumor morphology, location, pattern of enhancement and relation with adjacent structures. The signal intensities in T2WI were evaluated and compared with pathological subtypes. Results The tumors of 12 cases located in the clivus, 4 cases in other regions of skull base, including parasellar region (1 case) , jugular foramen region (1 case) and sphenoidal sinus (2 cases). All the tumors were regular or lobulated in morphology with well-defined margins with heterogeneous and honeycomb sign. TlWI demonstrated low or slightly low signal intensity in all tumors, but focal high signal intensity was seen in 8 cases. The tumors showed significantly high or slightly high signal intensity in T2WI. After injection of contrast, heterogeneous enhancement was detected in all lesions, including mild enhancement (4 cases), moderate enhancement (6 cases) and slowly progressive enhancement (6 cases). Among 16 cases, 10 cases were classic type, 6 cases were chondroid type, and no statistically difference of T2WI signal intensities was found between the two types. Conclusion On MR images, chordoma in the skull base has specific characteristics in the location, signal intensity and the pattern of enhancement. The pathologic type of chordoma can not be determined only according to T2WI signal intensities.
