Damon Ⅲ矫治器拔牙矫治软硬组织变化的研究

摘　　要：目的研究Damon Ⅲ矫治器一步滑动法关闭拔牙间隙，研究治疗前后牙验结构及软组织的变化。方法选择双牙弓前突或上牙弓前突的安氏Ⅱ类Ⅰ分类患者20人，平均年龄16．6岁。拔除四个第一双尖牙，应用美国Ormco公司Damon Ⅲ矫治器进行矫治，对治疗前后头颅侧位片进行18个项目的测量，并对测量结果进行配对t检验。结果治疗后上下切牙切缘分别向后移动7．47mm、4．93mm，根尖分别向后移动1．15mm、3．00mm，差异有统计学意义（P〈0．01）；上下磨牙分别向前移动1．82mm、1．90mm，差异有统计学意义（P〈0．01）。U1／SN、U1／PP角在治疗后分别减小了16．70°和15．91°，L1/MP角、上下唇——审美平面距在治疗后也明显减小，Z角增大，差异有统计学意义（P〈0．01）；而SNA、SNB、ANB、MP／SN角在治疗前后差异无统计学意义。结论应用Damon Ⅲ矫治器矫治双牙弓前突患者，在没有借助口外力及其它附加装置的条件下，仍可以进行很好的支抗控制，上下前牙回收明显，软组织侧貌改善显著，但上前牙的转矩控制可能不足。Objective To investigate the changes in cranio-facial hard and soft tissue of patients treated with Damon Ⅲ bracket by extraction of four first bicuspids. Methods Totally 20 patients aged 12 - 26 years were included in this study（3 males, 17 females, average age was 16. 6 years）. Four first premolars were extracted and Damon Ⅲ appliances manufactured by Ormco were bonded in all patients. One-step sliding technique was used for space closure and 18 cephalometric measurements were used for assessment. Paired t test was used in statistical analysis. Results After treatment, the edges of the upper and lower incisors were retruded for 7.47mm and 4. 93mm, and the apexes of the upper and lower incisors were retruded for 1. 15 mm and 3. 00 mm, and the significant difference was founded （P〈0. 001）. The upper and lower molars were protruded for 1.82 mm and 1.90 mm respectively and the difference was significant （P〈0. 001 ）, too. U1/SN, U1/PP angle decreased significantly by 16.70°and 15.91°respectively （P〈0. 001）. Furthermore, there were remarkable decrease on 1.1/MP angle, lip to E-line distance, and significant increase on Z angle （P〈0. 001）, too. There were no significant changes in SNA, SNB, ANB and MP/SN angle before and after treatment. Conclusions Profile can be improved significantly in bimaxillary protrusion patients treated with Damon Ⅲ bracket by removal of four first premolars. Anchorage can be managed successfully without facebow and any other additional appliances. Anterior teeth can be retracted significantly and posterior teeth move forward a little. There may be deficiency in lingual root torque control of upper incisors with Damon Ⅲ self-ligating brackets.
