单纯结膜转位术与联合视原胶原植入术治疗翼状胬肉的疗效观察

摘　　要：目的对翼状胬肉切除行结膜转位与联合视原胶原植入治疗翼状胬肉的临床效果进行观察和评价。方法对我院就诊的翼状胬肉患者56例(60只眼)随机分为A组和B组,A组采用翼状胬肉切除联合结膜转位术,B组进行翼状胬肉切除联合结膜转位及视原胶原植入术,对术后的炎症反应、并发症、视力、眼压、翼状胬肉复发率等进行了动态观察。结果术眼均未见排斥反应及并发症,随访观察11个月,B组有3只眼复发,复发率为10%;A组有5只眼复发,复发率为16.7%。两组比较差异无统计学意义,B组复发程度轻于A组。结论翼状胬肉切除行结膜转位与联合视原胶原植入术均可以有效地降低术后的复发程度,联合视原胶原植入术可减轻术后复发程度,是手术治疗翼状胬肉的一种安全、有效的方法。Objective To observe and evaluate the efficacy of conjunctival translocation and with porous collagen matrix transplantation after excision of pterygium. Methods 60 eyes in 56 patients with pterygium were studied. All eyes were randomly devided into group A and group B. The group A eyes underwent pterygium excision with conjunctival translocation , group B eyes were implanted porous collagen matrix after the same surgery. We observed the inflammatory reaction, rejection, visual acuity,introocular pressure, and the rate of recurrence after surgery. Results No significant complications were encountered. No obvious rejection reactions were demonstrated in all eyes. During the 11 months follow-up, pterygium recurred in 3 eyes（ 10% ） in group B and 5 eyes（ 16.7% ） in group A. No statistical difference was found between them. 3 recurrent pterygiums didn＇t exceed 1/2 range of the original in group B. Conclusion This study indicates that pterygium excision with conjunetival translocation and porous collagen matrix transplantation can reduce the rate of recurrence and it is a safe and effective technique in pterygium surgery.
