前列腺外周带癌灶的ADC值定量分析

摘　　要：目的:运用弥散加权对前列腺外周带癌灶的ADC值进行定量分析,初步提出外周带癌灶ADC值的参考值范围。方法:测量和分析78例正常前列腺组(正常组)和57例前列腺癌组(PCa组)于b值分别为500、800、1000s/mm2时外周带兴趣区和外周带癌灶的ADC值,对所得数据进行统计学分析。结果:不同b值下正常组外周带平均ADC值分别为(1.744±0.265)×10-3、(1.634±0.235)×10-3、(1.545±0.192)×10-3mm2/s,PCa组外周带癌灶的平均ADC值分别为(0.907±0.167)×10-3、(0.819±0.136)×10-3、(0.768±0.143)×10-3mm2/s。2组比较差异有统计学意义(P<0.05)。不同b值下PCa组外周带癌灶ADC值参考范围分别为(0.581,1.234)×10-3、(0.553,1.086)×10-3、(0.487,1.049)×10-3mm2/s。结论:通过ADC值测量可以定量分析前列腺外周带癌,结合常规MRI,可以提高前列腺癌影像诊断的符合率和特异性。Objective ：To quantitatively analyze ADC values of prostate cancerous, and to propose ADC reference range of prostate cancerous. Methods： ADC values in 78 volunteers and 57 patients with prostate cancerous confirmed by pathology in different b-values （500, 800, 1 000 s/mm^2） were quantitatively analyzed. ADC values in two groups were retrospectively measured. Results： ADC value in different b-values in normal group was（1.744±0.265）×10^-3 mm2/s,（1.634±0.235）×10^-3 mm^2/s, and （1.545±0.192）×10^-3mm^2/s respectively. It was （0.907±0.167）×10^-3 mm^2/s, （0.819±0.136）×10^-3 mm^2/s, （0.768±0.143）×10^-3mm^2/s in prostate cancer group respectively.The average ADC values in prostate cancer group were significantly lower than that in normal groups （P 〈 0.05）. ADC reference range of prostate cancerous in different b-values were （0.581,1.234）×10^-3, （0.553,1.086）×10^-3, （0.487,1.049）×10^-3 mm2/s. Conclusion： Quantitative evaluation of ADC with MRI test in prostate cancerous patients can improve the coincidence rate and specificity of prostate cancerous image diagnosis.
