T淋巴瘤侵袭转移诱导因子1表达水平在胃癌淋巴结转移中的意义

摘　　要：目的定量检测T淋巴瘤侵袭转移诱导因子1(Tiam1)在胃癌组织中的表达水平,探讨并评价其在胃癌淋巴结转移中的作用。方法采用TaqMan探针实时荧光定量PCR方法,定量检测66例胃癌患者癌组织、癌旁组织及11例胃良性病变组织中的Tiam1 mRNA拷贝数,同时将CK18作为淋巴结微转移的标志基因,定量检测24例病理诊断无淋巴结转移的胃癌患者199枚淋巴结的CK18mRNA拷贝数。结果66例胃癌患者的癌组织92.42%(61/66)Tiam1mRNA表达阳性,而癌旁为19.70%,胃良性病变组织11例只检测到1例Tiam1 mRNA表达,胃癌组织Tiam1mRNA阳性率高于癌旁组织(2χ=70.86)和良性病变组织(2χ=41.74)(均P<0.01),差异有统计学意义(χ2=78.96,P<0.01)。胃癌组织中Tiam1 mRNA表达量明显高于癌旁和良性病变组织(4.92×105vs.0)。66例患者根据淋巴结转移分为3组;24例病理无淋巴结转移的胃癌患者通过淋巴结CK18mRNA的定量检测,15例存在微转移。微转移组(9.51×104)及病理转移组(2.99×106)Tiam1 mRNA表达量明显高于未转移组(4.54×102,P<0.01)。结论Tiam1与胃癌淋巴结转移密切相关。胃癌组织Tiam1表达水平可能决定癌细胞淋巴转移的倾向,可以成为术前评估转移的实验诊断参考指标。Objective To detect quantitatively the expression level of T lymphoma invasion and metastasis inducing factor 1 （Tiara1） in gastric carcinoma tissues, and evaluate the role of Tiara1 in lymph node metastasis of gastric carcinomas. Methods Real-time fluorescence quantitive RT-PCR （RT-FQ-PCR） was used to detect the expression level of Tiam1 mRNA in gastric cancer tissues of 66 cases, para-cancerous tissues （over 3 cm） of 66 cases and gastric benign lesion tissues of 11 cases. Meanwhile, CK18 was selected as the hallmarker for lymph node micrometastasis to detect the level of CK18 mRNA expression in 199 lymph nodes from 24 cases of gastric carcinomas diagnosed negative by pathologic examinations. Results The positive rate of Tiam1 mRNA expression in gastric cancer tissues and para-cancerous tissues was 92.42% （61/66） and 19.70% （13/66）, respectively, and only one case in 11 gastric benign lesion tissues expressed Tiam1 mRNA. There were significant differences （P〈0.01） among the three groups of patients. The positive expression rate of Tiam1 mRNA in gastric cancer tissues was much higher than that in the para-cancerous tissues （ χ^2 =70.86） and gastric benign lesion tissues （ χ^2 =41.74） （P〈0. 01）. The expression level of Tiam1 mRNA in gastric cancer tissues was much higher than that in the para-cancerous tissues and gastric benign lesion tissues （4.92 × 10^6 vs. 0）. The 66 cases were divided into three groups according to the degree of lymph node metastasis. 15 cases had micrometastasis, 24 cases of gastric carcinomas were diagnosed negative by pathologic examinations. The level of Tiam1 expression in micrometastasis group （9. 51 × 10^4） and metastasis pathologically group （2.99 × 10^6 ） was much higher than that in non-metastasis group （4.54 ×10^2 , P〈0. 01）. Conclusion There is a close correlation between Tiam1 and lymph node metastasis of gastric carcinomas. The expression level of Tiam1 in gastric carcinomas probably determines the metastasis tenden
