摘 要:目的探讨HPA-1—6、15等位基因与特发性血小板减少性紫癜(ITP)的相关性。方法采用聚合酶链反应序列特异性引物(PCR-SSP)法,对ITP组(n=25)和健康对照组(n=25)进行HPA-1—6、15共7个抗原系统基因分型,采用ELISA法和微柱凝胶法检测2组人群血小板吸附抗体及血小板游离抗体。结果ITP组HPA-3的基因频率明显异于对照组(P<0.01),其中HPA-3 a基因频率高于对照组,HPA-3b基因频率低于对照组,其余等位基因在2组间未发现有明显区别(P>0.05);ITP组血小板游离抗体检测到17例阳性(68%),吸附抗体检出21例(84%),对照组血小板抗体未检出。结论本次研究显示HPA-3抗原系统和ITP的发病可能存在一定的关联;血小板吸附抗体在ITP中检出率明显高于游离抗体。Objective To observe the relationship between HPA-1--5, 6,15 alleles and idiopathic thrombocytopenic purpura. Methods Polymerase chain reaction-specific sequence primers (PCR-SSP) was used to gcnotype HPA-1-5, 6, and 15 in the ITP group (n = 25 ) and the health control group (n = 25). The platelet adsorbed and free antibodies in the two groups were detected by ELISA and microcolumn gel immunoassay. Results The allele frequencies of HPA-3 was significantly different between the ITP group and control group (P 〈0.01), and the frequency of HPA-3a in ITP group was higher than that in control group, and the frequency of HPA-3b was lower than that in the control group, the frequencies of other alleles showed no significant difference between the two groups( P 〉 0. 05 ). The platelct free antibodies were detected in 17 ITP samples(68% ) and the adsorbed antibodies were detected in 21 ITP samples(84% ). No platelet antibodies were detected in the control group. Conclusion There was a possible association between the HPA-3 allele and ITP. The detection rate of platelet adsorbed antibodies is significant higher than that of platelct free antibodies.
