视网膜全脱离超声诊断的声像图特征探讨

摘　　要：目的探讨视网膜全脱离超声诊断的声像图特征。方法对比分析经临床手术、三面镜等其他检查证实的32例(32眼)视网膜全脱离患者与21例(22眼)部分性视网膜脱离患者的声像图资料,总结准确诊断视网膜全脱离的声像图特征。结果(1)眼轴位声像图的标准显示是诊断视网膜全脱离的基本要求。(2)玻璃体内两端固定的、顶端连于视乳头的"V"形或"Y"形稍强回声带是诊断视网膜全脱离的声像图特征之一。(3)以眼轴位为中心的2个十字交叉的切面声像图的同时显示是诊断视网膜全脱离范围必要的补充手段。(4)视网膜脱离的非视乳头端超过眼球赤道部是反映视网膜全脱离程度的标志。结论不能简单地将玻璃体内两端固定的、顶端连于视乳头的"V"形或"Y"形稍强回声带作为准确诊断视网膜全脱离的声像图特征,而应将其眼轴位声像图的标准显示和结合以眼轴位为中心的2个十字交叉的切面声像图以及视网膜脱离的非视乳头端的附着点共同作为准确诊断视网膜全脱离的附加要求。Objective To investigation the significance of ultrasonography in the diagnosis of totally detached retina. Methods The ultrasonographic data were reviewed in 32 patients （32 eyes） with total detachment of retina and in 21 patients （22 eyes） with partial detachment of retina confirmed by surgery and ophthalmological examination, so that the uhrasonographic significance in the diagnosis of totally detached retina would be obtained. Results （ 1 ）To show the section of axis oculi exactly was the basic demand in diagnosing totally detached retina. （;2） The ＂V＂ or ＂Y＂ hyper-echo zone between the papilla optica and para-lens was of ultrasonographic significance in making the diagnosis of totally detached retina. （3） It was necessary to show ＂ V＂ or ＂ Y ＂ hyper-echo zone between the papilla optica and para-lens with two section crossing each other. （4） The non-papilla optica point of the detached retina could reflect the degree of the totally detached retina. Conclusion When we are to make a diagnosis of total detachment of retina, we should show clearly the section of axis oculi and add up to the two section crossing and the non-papilla optica point of the detached retina. We should not simply depend on the ＂V＂ or ＂Y＂ hyper-echo zone between the papilla optica and para-lens.
