GuttaFlow结合不同锥度牙胶尖充填质量的体外评价

摘　　要：目的:对常温牙胶充填系统GuttaFlow采用不同锥度牙胶尖充填质量进行体外评价。方法:将40颗上颌切牙经K3预备至0.06锥度40号后,随机分为0.02锥度组和0.06锥度组,分别用多根0.02锥度牙胶尖和单根0.06锥度牙胶尖,结合GuttaFlow进行根管充填。使用RVG摄像系统,对每颗牙颊舌向、近远中向进行X光投照后评分。包埋冠部,自根尖向冠方垂直于牙体长轴每隔1mm制备横截面切片。体视显微镜下观察切片中空隙的分布与面积。结果:0.02锥度组近远中向及双向整体评分、空隙的发生率及距根尖5～8mm水平的空隙横截面数均显著高于0.06锥度组;其他观察指标两组之间无显著性差异。近远中向评分与空隙面积百分比显著相关。结论:使用GuttaFlow系统进行根充时,推荐采用与根管预备锥度相一致的主牙胶尖,并且根管上段可适当增加辅尖。Objective：To compare the of root fillings quality of different tapered gutta-percha cones combined with GuttaFlow obturation system. Methods：40 extracted human maxillary incisors were prepared with K3 Ni-Ti rotary system to file size 40 and 0.06 taper. The teeth were randomly separated into 0.02 tapered group and 0.06 taper group. These teeth were respectively obturated with multiple 0.02 tapered gutta-percha points or a single 0.06 tapered gutta-percha point using GuttaFlow system. Each root canal was radiographed in the bucco-lingual （BL） and the mesio-distal projection （MD） with RVG system,and evaluated according to a scoring system. And then roots were sectioned at 1mm interval from apex to crown. The number of surfaces with voids,the canal area and the void area were measured. The percentage of void area （PV） was calculated. Results：There were statistically significant differences in the parameters as follows：radiographic scores of MD,combined MD and BL scores,the frequency of surfaces with voids,and the number of sections with voids at 5-8mm levels of 0.02 taper group were higher than those of.06 taper group. It showed no significant differences between the other parameters of the two groups. The correlation between MD scores and PV was significant. Conclusion：Better quality of root fillings was obtained with the same prepared taper gutta-percha cones when using GuttaFlow system,and additional gutta-percha points were recommended to obturate the coronal parts.
