摘 要:目的观察碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)与异种脱钙骨复合修复骨缺损时骨修复效果及血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)的表达情况。方法在15只成年新西兰兔双侧下颌骨下缘形成15mm×5mm全层骨膜骨质缺损,每一缺损作为1个实验单位,按自身同期配对设计,分别植入复合骨(实验组)和单纯异种脱钙骨(对照组)。术后2、4和8周取材作组织学检查,采用图像分析软件量化计算成骨量和VEGF蛋白表达水平,并分析其相互关系。结果术后2、4和8周实验组与对照组相比,成骨量分别增加193.9%、160.1%和124.5%,VEGF的蛋白表达相应地增加424.4%、540.5%和290.2%,VEGF表达和新骨形成呈密切相关(r=0.94,P<0.0001)。结论bFGF复合的异种脱钙骨可显著地上调骨移植区VEGF的表达,bFGF通过上调VEGF的表达以促进血管化从而增强异种脱钙骨的愈合。Objective To study the correlation between the level of vascular endothelial growth factor (VEGF) expression and the amount of newly formed bone in repairing mandible defects in rabbits with demineralized bone xenograft combined with basic fibreblast growth factor(bFGF). Methods 15 mm × 5 mm bilateral mandibular periosteum bone defects were made surgically in 15 New Zealand adult rabbits; the right defects were repaired with demineralized bone xenograft combined with bFGF at 10 ng/mm^3 as experimental groups; the left defects were repaired with demineralized bone xenograft as controls. Specimens were obtained and processed for histological and immunohisteehemical examinations 2,4,8 weeks after operation respectively. The bone formation was evaluated grossly and histologically, and the amount of new bone and the percentage of positive staining area were quantified by image analysis. Then the correlation between levels of VEGF production and the amount of newly formed bone was also analyzed by statistics software. Results The amount of new bone formed in defects grafted with composite bFGF/demineralized bone xenograft was 193.9%, 160. 1% and 124.5% more than that formed with demineralized bone xenograft alone 2,4, 8 weeks after operation respectively, and the level of VEGF expression increased by 424.4%, 540.5 % and 290.2% accordingly. A close correlation existed between VEGF expression production and the amount of newly formed bone ( r = 0.94, P 〈 0. 0001 ). Conclusions bFGF reconstituted in demineralized bone xenograft markedly up - regulated the expression of VEGF in the grafted area. bFGF augments the healing of demineralized bone xenograft by enhancing vascularization through the up - regulation of VEGF.
