摘 要:目的比较产科超声检查中二维(2D)与四维(4D)超声能量输出指数(机械指数和热指数),探讨此二种超声扫查方式对胎儿的影响。方法在检查100例中晚孕胎儿的过程中,首先用2D常规检查胎儿,记录2D检查时的热指数(TI)和机械指数(MI)显示数值,然后记录检查所花时间,随即改为4D扫查,主要观察胎儿的颜面部,同时记录检查时的TI及MI数值,最后记录检查所用时间。结果100例中晚孕胎儿,平均妊期为31.6周,2D的TI和MI分别为0.45±0.05和0.93±0.09,4D的TI和MI分别为0.66±0.15和0.83±0.08;2D检查时间为4.62±1.06min;4D检查时间为3.52±1.11min;2D+4D的检查时间为8.22±1.54min。结论平均的2D超声扫描MI较4D的高,平均的4D超声扫描TI较2D的高;在2D扫查时间基础上所增加的4D扫查时间延长了胎儿的超声辐照,这是否会对胎儿造成一定的生物学损害有待进一步实施动物实验和临床研究。Objective To compare the acoustic output indices, as expressed by thermal index (TI) and mechanical index (MI) in 2D and 4D ultrasound in obstetrics and discuss the effect of these two ultrasound modes on fetuses. Methods 100 second and third trimester pregnant women were firstly examined with 2D ultrasound, acoustic output indices(TI and MI) and examination duration were recorded, then examined with 4D ultrasound, acoustic output indices(TI and MI) and examination duration were also recorded. Results Mean gestational age was 31.6 weeks, mean TI and MI in 2D mode were 0.45±0.05 and 0.93±0.09, mean TI and MI in 4D mode were 0.66±0.15 and 0.83±0.08; mean exam duration for 2D ultrasound was 4.62±1.06 min, mean exam duration for 4D ultrasound was 3.52±1.11 rain, while mean exam duration for 2D + 4D ultrasound was 8.22±1.54 min. Conclusion Mean MI of 2D ultrasound was higher than that of 4D ultrasound, mean TI of 4D ultrasound was higher than that of 2D. The additional time of 4D examination prolonged the whole actual scanning time, thus raising a potential hazard to fetuses. An animal experiment is needed to prove this.
