立普妥治疗40例颈动脉粥样硬化斑块临床观察

摘　　要：目的:本研究旨在较长期随诊观察立普妥(阿托伐他汀钙)对脑梗死患者颈动脉粥样硬化斑块的治疗作用。方法:将脑梗死住院患者74例随机分为治疗组和对照组,治疗组给予阿托伐他汀20mg/d,对照组采用饮食控制或服用烟酸、脂脉康胶囊等非他汀类药物。分别于入院时、治疗第3个月、6个月、12个月动态监测两组患者的血脂水平,并进行颈动脉超声检查评估颈动脉斑块积分。比较两组患者血脂和颈动脉斑块积分的变化。结果:共有74例患者最终完成了12月的随诊观察。其中阿托伐他汀治疗组40例,对照组34例,两组间血脂变化差异无显著性(P>0.05),而颈动脉斑块积分在治疗6个月以后组间差异有显著性(P<0.05),两组颈动脉斑块积分(随诊6个月后)与初诊时相比也有显著性差异(P<0.05),治疗组斑块积分下降,而对照组积分增加。阿托伐他汀治疗组未发现有严重不良反应。结论:阿托伐他汀除有良好的降脂作用外还有延缓甚至减轻动脉粥样硬化的作用,如无严重不良反应应坚持长期用药。Objective：To observe the effect of atorvastatin on carotid atherosclerosis in cerebral infarction patients.Methods：74 cases patients with cerebral infarction were divided into two groups randomly.40 patients were treated with atorvastatin（20mg）per night for 12 months. The rest of patients were treated without atorvastatin, and considered as control group. The fasting blood lipid level and carotid artery plaques score were measured at the beginning of the study （as baseline）and 3,6,12month after treatment in both groups. Results ： There was no significant difference between the two group on the levels of blood Tch ,TG,I-IDL,and LDL at all time points（ P 〉0.05）.However,there is statistically significant difference on the carotid artery plaque score between treated and untreated groups at both 6 and 12- month time points. In atorvastatin- treated group, the plaque scores of the patients were decreased while those of control group showed an increased level（ P 〈 0. 05）. Conclusion：This study suggested that atorvastatin could possibly delay the progression of carotid artery atherosclerosis in addition to their lipid - lowering effects.
