摘 要:背景与目的:目前研究认为,TGF-β/Smads信号通路在恶性肿瘤的发生发展过程中起到重要的作用,并可能与胃癌等恶性肿瘤的生物学行为密切相关。本研究通过探讨TGF-βRⅡ、Smad4和Smad7在胃癌组织的表达及其意义,分析它们与临床病理特征及患者生存之间的关系。方法:应用组织芯片技术和免疫组化SABC法检测200例胃癌组织及56例癌旁组织TGF-βRⅡ、Smad4和Smad7蛋白的表达。结果:胃癌组织中TGF-βRⅡ、Smad4和Smad7的阳性率分别为25.5%、67.0%和47.0%。TGF-βRⅡ表达与胃癌浸润深度、淋巴结转移、分化程度和Lauren分型密切相关(分别χ2=6.214,χ2=11.308,χ2=14.633和χ2=8.216,均P<0.05);Smad4表达与胃癌分化程度及Lauren分型密切相关(分别χ2=16.162和χ2=13.100,均P<0.05);Smad7表达与胃癌分化程度及Lauren分型密切相关(分别χ2=4.710和χ2=5.297,均P<0.05);Smad4与TGF-βRⅡ表达呈正相关(r=0.191,P=0.007);Smad4表达与患者生存密切相关(χ2=4.090,P=0.043)。结论:胃癌组织中广泛存在TGFβ-Smad信号通路相关分子TGF-βRⅡ、Smad4和Smad7表达异常。胃癌中Smad4蛋白阳性患者其预后优于阴性者。Background and Objective. Recent studies have revealed that TGF-β/Smads signaling pathway plays a pivotal role in the oncogenesis and development of malignant tumors, and may closely relate to the biological behaviors of some malignant tumors, such as gastric carcinoma. This study was to investigate the expression of transforming growth factor-β receptor II (TGF-βRⅡ ), Smad4 and Smad7 proteins in gastric carcinoma, and explore their correlations to clinicopathologic characteristics and prognosis of gastric carcinoma. Methods: The expression of TGF-βRⅡ , Smad4, and Smad7 was detected by SABC immunohistochemistry and tissue microarray which containing 200 specimens of primary human gastric carcinoma and 56 specimens of adjacent gastric tissue. Results:The positive rates of TGF-βRⅡ , Smad4, and Smad7 in gastric carcinomas were 25.5%, 67.0%, and 47.0%, respectively. TGF-βRⅡ xpression was related with depth of invasion, lymph node metastasis, tumor differentiation, and Lauren classification (X2=6.214, X2=11.308, X2=14.633, and X2=8.216, respectively, all P〈0.05). Smad4 expression was related with tumor differentiation and Lauren classification (X2=16.162 and X2=13.100, all P〈0.05). Smad7 expression was related with tumor differentiation and Lauren classification (X2=4.710 and X2=5.297, all P〈 0.05). Smad4 expression was positively correlated to TGF-βRⅡ expression (r = 0.191, P=0.007). Smad4 expression was related with patients' survival (X2= 4.090, P=0.043). Conclusions: Abnormal expression of TGF-Smad signaling pathway proteins occurs in gastric carcinoma. Smad4-positive gastric carcinoma patients have better prognosis than Smad4-negative patients.
