全直肠系膜切除及双吻合器技术在中低位直肠癌保肛手术中的应用

摘　　要：目的：评价全直肠系膜切除（TME）、双吻合器技术（DST）在中低位直肠癌保肛手术中的应用。方法：回顾性分析36例采用直肠全系膜切除、双吻合器行低位直肠癌前切除术患者临床资料。结果：术后发生吻合口漏2例，吻合口狭窄1例。术后随访1～2年，局部复发2例，远处转移和死亡2例。结论：中低位直肠癌全直肠系膜切除能降低局部复发，而双吻合器的应用能提高低位直肠癌的保肛率。Objective ：To evaluated the values of both the total mesorectal excision （TME） and the double staping technique （ DST in middle or low rectal carcinoma. Method. The author summarized 36 patients with low rectal carcinoma had undergone rectectomy with this technique. Results：The anastomotic leakage occurred in 2 patients, and anastomotic stricture occurred in 1 cases in this group. All patients were followed - up for 1 to 2 years, local recurrence occurred in 2 cases, distance metastases and death occurred in 2 patients. Conclusion： TME can decrease the rates of local recurrence in low rectal carcinoma,and the double stapling technique can increase the probability of anus reservation.
