中国北方地区肝硬化及肝癌患者乙型肝炎病毒基因型及亚型分析

摘　　要：目的了解中国北方地区乙型肝炎病毒(HBV)基因型及亚型的分布情况及其特点,探讨HBV基因亚型与终末期肝病的相关性。方法选择中国北方地区HBV感染的肝硬化标本101例、原发性肝癌标本178例,应用巢式聚合酶链反应对HBV DNA进行基因型及亚型检测,并结合临床资料分析HBV基因亚型与终末期肝病的相关性。结果在279例HBV感染者组织或血清样本中,HBV Ba基因亚型7例(2.5%),C2亚型253例(90.7%),Ba和C2混合型19例(6.8%),未发现其他基因型。肝硬化和肝癌组织中HBV基因型及亚型分布的差异无统计学意义(P=0.301)。在肝硬化患者中各种基因亚型的乙型肝炎e抗原(HBeAg)阳性率(P=0.618),丙氨酸转氨酶(ALT)水平(P=0.692),总胆红素(TBIL)水平(P=0.165),差异均无统计学意义,在肝癌患者中各种基因亚型的HBeAg阳性率(P=0.563),ALT水平(P=0.940),TBIL水平(P=0.313)差异亦无统计学意义。结论HBV基因型在北方肝硬化和原发性肝癌患者中以C2亚型为主。Objective To investigate the distribution of hepatitis B virus（HBV） genotypes and subgenotypes in northern China,and determine the relationship between the subgenotypes of HBV and serious hepatitis B. Methods HBV genotypes and subgenotypes of 101 liver cirrhosis（LC） and 178 hepatocellular carcinoma（HCC） cases were determined with nested PCR, and the relevance between HBV subgenotypes and serious hepatitis B was statistically analyzed. Results Of 279 tested specimens,7（2.5%） were Ba subgenotypes,253（90.7%） were C2 subgenotypes,and 19（6.8 %） were Ba and C2 subgenotypes co-infection. No other genotypes or subgenotypes were found. No significant distribution difference of HBV genotypes and/or subgenotypes was found between liver cirrhosis and HCC cases. And no significant difference proved between HBeAg positive rate, and ALT, TBIL level of liver cirrhosis（LC） or HCC patients with different genotypes or subgenotypes. Conclusion Subgenotype C2 is dominant in northern China in the patients with LC and HCC infected with different HBV genotypes and/or subgenotypes.
