摘　　要：目的:观察FOLFOX4方案治疗晚期胃肠癌的临床疗效和耐受性。方法:54例晚期胃肠癌患者接受FOLFOX4方案化疗,即:奥沙利铂85mg/m2,静脉滴注2h,d1;甲酰四氢叶酸钙200mg/m2,静脉滴注2h,d1、d2;氟尿嘧啶400mg/m2,快速静脉滴注,dl、d2,600mg/m2,持续静脉滴注22h,d1、d2,每2周重复。结果:全组54例均可评价疗效,总有效率42.6%(23/54)。其中,初治24例,CR4例,PR10例,有效率58.3%;复治30例,CR2例,PR7例,有效率为30.0%;初治与复治的疗效比较有统计学意义(P<0.05)。毒副反应主要为胃肠道反应、周围神经毒性及骨髓抑制等,无治疗相关性死亡。结论:FOLFOX4方案为治疗晚期胃肠癌的有效方案,毒副反应较轻,患者耐受良好。Objective： To observe the clinical effect and tolerance of using oxiplatin（L-OHP） in combination with infusional 5 -fluorouracil （ 5 - FU） and calcium folinate （CF） （ FOLFOX4 regimen） in treatment of advanced gastrointestinal cancer. Methods： A total of 54 advanced gastrointestinal cancer patients received FOLFOX4 regimen by administering intravenously oxaliplatin 85 mg/m^2 on day 1, CF 200 mg/m^2 as a 2h infusion followed by bolus 5 -FU 400 mg/m^2 and a 22h infusion of 5 -FU 600 mg/m^2 , with repetition for 2 consecutive days every 2 weeks. Results ： The evaluation for the efficacy in the total 54 patients suggested that 42.6% （23/54） responded to the drug therapy. Among the 24 patients received first-line chemotherapy, 58.3% responded to the regimen, in which 4 cases were observed with complete response and 10 with partial response. For 30 patients undergone second-line chemotherapy, the response rate was 30.0% （ CR in 2, PR in 7）. The response rate of first-line chemotherapy was significantly better than that of second-line chemo-therapy（ P 〈 0. 05 ）. The adverse reaction was mainly involved in gastrointestinal reaction, peripheral neuropathy and marrow suppression. No death resulted from the regimen. Conclusion： The FOLFOX4 regimen was effective in advanced gastrointestinal cancer, with less adverse effect and excellent tolerance.
